目的应用螺旋CT评价阻塞性睡眠呼吸暂停综合征(OSAS)患者咽壁软组织和上气道狭窄情况及形态特点。方法经多导睡眠监测仪(PSG)诊断的OSAS患者7例均行清醒状态和深睡状态下上气道软腭后区至会厌后区螺旋CT扫描。结果①5例发生完全性阻塞,且阻塞部位均为软腭后区,有2例分别为多部位阻塞;②OSAS患者软腭长度44.28mm±1.16mm(正常长度41.90mm±1.95mm),软腭厚度9.42mm±0.46mm(正常厚度5.84mm±0.24mm);③OSAS患者在清醒及睡眠状态下软腭后区(RP)、舌后区(RG)、会厌后区(EPG)咽后壁厚度均大于正常值。结论螺旋CT对OSAS临床综合诊断及病因研究具有较高的实用价值。
Objective To evaluate the characteristics of pharyngeal wall soft tissue and upper airway stenosis in patients with obstructive sleep apnea syndrome (OSAS) by spiral CT. Methods Seven patients with OSAS diagnosed by polysomnography (PSG) underwent spiral CT in the posterior parietal region of the upper airway to the posterior region of the epiglottis under awake and deep sleep. Results ① Complete occlusion occurred in 5 cases, and the posterior part of the soft palate was obstructed in 2 cases, with multiple sites obstructed in 2 cases. ② The length of soft palate in OSAS patients was 44.28mm ± 1.16mm (normal length 41.90mm ± 1.95mm) and soft palate thickness 9.42mm ± 0.46mm (normal thickness 5.84mm ± 0.24mm). ③ The posterior parietal area (RP), posterior lingual area (RG) and epiglottis area (EPG) posterior pharyngeal wall thickness were all greater than normal in patients with OSAS under awake and asleep conditions. Conclusion Spiral CT has great practical value in the clinical diagnosis and etiology of OSAS.
