目的探讨超声弹性成像对新版超声乳腺影像报告和数据系统(BI-RADS-US)评估乳腺肿物良恶性的辅助价值。方法回顾性分析已行常规超声(US)检查和弹性成像(UE)检查的862个乳腺肿物,结合病理结果,观察UE对旧版和新版BI-RADS评估乳腺肿物的辅助价值。结果当4类病变未被亚分类时,US、UE以及结合组的ROC曲线下面积(AUC)分别为0.735、0.877、0.878(P<0.01)。而当4类病变被亚分类后,US、UE及结合组的AUC分别为0.865、0.877、0.883(P>0.05)。结论按旧版BI-RADS评估US时,UE有助于提高US对乳腺病变的诊断效率;而按新版BI-RADS评估US时,UE与US的诊断效率相当,但结合UE能减少对4A类乳腺肿物不必要的术前活检。
Objective To investigate the value of ultrasound elastography in the evaluation of benign and malignant breast masses by the new edition of Breast Imaging Report and Data System (BI-RADS-US). Methods We retrospectively analyzed 862 breast masses that underwent conventional US and elastography. Combined with the pathological results, we observed the value of UE in evaluating the value of breast tumor in both old and new versions of BI-RADS. Results When the four lesions were not classified, the area under the ROC curve (AUC) of US, UE and the combination group were 0.735, 0.877 and 0.878 (P <0.01), respectively. When the four types of lesions were sub-classified, the AUC of US, UE and combination group were 0.865, 0.877 and 0.883, respectively (P> 0.05). CONCLUSIONS: UEs can help to improve the diagnostic efficiency of breast cancer in US based on the previous version of BI-RADS. However, when the new version of BI-RADS is used to evaluate US, the diagnostic efficiency of UE is similar to that of US. However, Tumor unnecessary preoperative biopsy.
