目的 探讨乳腺癌和纤维腺瘤动态增强MR对比剂空间分布及其与微血管密度 (MVD)的相关性。资料与方法 对 6 3例乳腺肿瘤术前行MR平扫和动态增强扫描 ,分析瘤中心、瘤边缘和瘤旁MR对比剂空间分布情况。术后标本行免疫组织化学染色 ,测定肿瘤MVD及分布情况 ,并分析与相应病灶MR对比剂空间分布之间的关系。结果 乳腺癌与纤维腺瘤内部MR对比剂空间分布有显著性差异 ,乳癌组瘤边缘强化程度 (△SI)显著高于瘤中心 ;而腺瘤组瘤边缘△SI显著低于或等于瘤中心。乳癌组瘤中心、瘤边缘和瘤旁△SI显著高于腺瘤组。乳腺肿瘤动态增强MR对比剂空间分布与MVD呈显著正相关。结论 乳腺癌和纤维腺瘤动态增强MR对比剂空间分布显著不同 ,并且与MVD分布密切相关。
Objective To investigate the dynamic distribution of MR contrast agent in breast cancer and fibroadenoma and its relationship with microvessel density (MVD). Materials and Methods Sixty-three cases of breast tumors were scanned with MR plain and dynamic contrast-enhanced MR before operation. The spatial distribution of MR contrast agent in tumor center, tumor margin and para-tumor was analyzed. Postoperative specimens were examined by immunohistochemical staining to determine MVD and distribution of tumors, and to analyze the relationship between spatial distribution of MR contrast media and corresponding lesions. Results There was a significant difference in the spatial distribution of MR contrast agent between breast cancer and fibroadenoma. The enhancement of △ SI in breast cancer was significantly higher than that of the tumor. The △ SI of tumor margin in adenoma was significantly lower than or equal to the tumor center. Breast cancer tumor center, tumor margin and tumor side △ SI was significantly higher than the adenoma group. Dynamic enhancement of breast tumor MR contrast agent spatial distribution was positively correlated with MVD. Conclusion The dynamic contrast-enhanced MR imaging of breast cancer and fibroadenoma has significantly different spatial distribution and is closely related to the MVD distribution.
