脑胶质瘤尤其是低级别胶质瘤通常会以癫为起病症状,称为肿瘤相关性癫(TRE)。对TRE的治疗是神经外科面临的重大难题,目前对于TRE的发生和发展机制研究还不够透彻,对其的理解还不够深入。文内归纳了TRE可能机制的相关文献,包括肿瘤自身和肿瘤周边的因素、肿瘤部位、组织学类型、基因等方面的改变,肿瘤周边的神经递质、离子、受体、细胞因子、微环境改变都为TRE的发生发展创造了条件。脑神经网络的改变可以更好地解释癫发作对脑功能系统性变化的影响。了解这些机制将对TRE治疗有一定帮助。
Gliomas, especially low-grade gliomas, usually take epilepsy as their initial symptom, known as tumor-associated epilepsy (TRE). The treatment of TRE is a major problem in neurosurgery. At present, the research on the mechanism of the occurrence and development of TRE is not thorough enough and its understanding is still not deep enough. The article summarizes the relevant literature about the possible mechanism of TRE, including the tumor itself and tumor peripheral factors, tumor location, histological type, gene changes, neurotransmitter around the tumor, ions, receptors, cytokines, microenvironment Changes have created the conditions for the occurrence and development of TRE. Changes in the brain network can better explain the impact of epileptic seizures on systemic changes in brain function. Understanding these mechanisms will be of some help to TRE treatment.
