目的探讨不同扫描及重建方式对冠状动脉支架图像质量的影响。方法对6支不同类型及直径的冠状动脉支架,分批注入6种不同浓度碘对比剂(在120kVp条件下的CT值:A组,138HU;B组,186HU;C组,205HU;D组,250HU;E组,374HU;F组,414HU),将其置于水箱中,利用GE Discovery CT750HDCT机采用不同方式扫描及重建:单扇区扫描标准重建,单扇区扫描细节重建,高清扫描标准重建,高清扫描高清重建。采用4级评分法对图像质量进行主观评分。结果高清扫描高清重建时,各浓度组支架均显示清晰。两两比较,A~E组高清扫描高清重建方式与其他扫描重建方式组间差异均有统计学意义(P均<0.05);F组单扇区扫描细节重建方式与高清扫描高清重建方式图像质量评分差异无统计学意义(P>0.05)。高清扫描高清重建与其他两种扫描及重建方式图像质量评分比较差异均有统计学意义(P均<0.05)。单因素方差分析表明,高清模式下各浓度组间图像质量评分差异无统计学意义(P>0.05)。结论与其他扫描和重建方式比较,高清扫描高清重建对冠状动脉支架的显示较佳。但当管腔内对比剂浓度较高(CT值414HU)时,其与单扇区扫描细节重建方式之间对支架的显示无明显差别。对比剂的CT值在138~414HU之间时对高清模式评估支架无明显影响。
Objective To investigate the effect of different scanning and reconstruction methods on coronary stent image quality. Methods Six coronary stents of different types and diameters were injected with six different concentrations of iodine contrast agent (CT value at 120 kVp: group A, group 138HU, group B, group 186HU, group C, group 205HU, group D, 250HU; Group E, 374HU; Group F, 414HU) were placed in a water tank and scanned and reconstructed using the GE Discovery CT750HDCT machine in different ways: standard reconstruction of single sector scan, detail reconstruction of single sector scan, standard reconstruction of high definition scan , HD scan HD reconstruction. The 4-grade grading method was used to subjectively evaluate the image quality. Results High-resolution HD reconstruction, the concentration of each group showed clear stent. In any pairwise comparisons, there was significant difference between group A and group E (P <0.05). The detail reconstruction method of single-sector scan in group F and the image quality of HD reconstruction Score difference was not statistically significant (P> 0.05). There were significant differences in image quality scores between HD scan HD reconstruction and other two scanning and reconstruction methods (all P <0.05). One-way ANOVA showed that there was no significant difference in the image quality scores among different concentration groups in HD mode (P> 0.05). Conclusion Compared with other scanning and reconstruction methods, high-resolution HD reconstruction of coronary stent better. However, when the lumen contrast agent concentration is high (CT value of 414HU), there is no significant difference between the scaffold display and single-sector scan reconstruction method. Contrast agent CT values between 138 ~ 414HU had no significant effect on HD-mode evaluation of the stent.
