目的了解人免疫缺陷病毒(HIV)/乙型肝炎病毒(HBV)合并感染者的CD_4~+T淋巴细胞与其基线肝功能和乙肝病毒复制水平(HBV DNA)的关系。方法对准备开始高效抗逆转录病毒治疗(HAART)的HIV/HBV合并感染患者69例进行基线CD_4~+T淋巴细胞、HBV DNA和肝功能检测。结果1.CD_4~+T淋巴细胞≤100个/μL组患者的天门冬氨酸氨基转移酶(AST)、HBV DNA显著高于CD_4~+T淋巴细胞>100个/μL组(P<0.05);CD_4~+T淋巴细胞≤100个/μL组患者的血清白蛋白(Alb)、凝血酶原活动度(PTA)显著低于CD_4~+T淋巴细胞>100个/μL组(P<0.05);两组血清总胆红素(TBI)、血清直接胆红素(DBI)、谷氨酸氨基转移酶(ALT)、血清球蛋白(Glo)差异无统计学意义(P>0.05)。2.CD_4~+T淋巴细胞计数水平与AST,HBV DNA,Alb,PTA的相关系数分别为(-0.314,-0.344,-0.35,0.333,P<0.05)。结论随着患者AIDS病情进展,HIV/HBV合并感染者的HBV DNA复制水平增高,肝功能损害进行性加重。
Objective To investigate the relationship between CD_4 ~ + T lymphocytes and their baseline liver function and hepatitis B virus replication (HBV DNA) in patients with human immunodeficiency virus (HIV) / hepatitis B virus (HBV) infection. Methods Sixty-nine patients with HIV / HBV co-infection who were about to start HAART were enrolled in this study. Baseline CD_4 ~ + T lymphocytes, HBV DNA and liver function were measured. Results 1. Aspartate aminotransferase (AST) and HBV DNA in CD_4 ~ + T lymphocytes ≤100 cells / μL group were significantly higher than those in CD_4 ~ + T lymphocytes> 100 cells / μL group (P <0.05) (P <0.05). The serum albumin (Alb) and prothrombin activity (PTA) of patients with CD_4 ~ + T lymphocyte ≤100 / μL group were significantly lower than those of CD_4 ~ + T lymphocyte> 100 cells / The levels of total bilirubin (TBI), serum direct bilirubin (DBI), glutamate aminotransferase (ALT) and serum globulin (Glo) in the two groups had no significant difference (P> 0.05). The correlation coefficients of CD4 ~ + T lymphocyte count with AST, HBV DNA, Alb and PTA were (-0.314, -0.344, -0.35, 0.333, P <0.05) respectively. Conclusion With the progression of AIDS in patients, the level of HBV DNA replication in HIV / HBV co-infected patients increases, and the liver dysfunction worsens.
