目的:研究同步间歇指令通气+压力支持通气(SIMV+PSV)模式对慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者的治疗效果。方法:选取80例COPD合并呼吸衰竭患者,随机分为观察组和对照组,每组各40例,对照组以双水平气道正压通气(BiPAP)模式治疗,观察组以SIMV+PSV模式治疗,检测和比较两组患者治疗前与治疗后第7天肺功能、血气分析、氧代谢三组指标。结果:治疗前两组患者肺功能、血气分析、氧代谢指标的差异均无显著性(P>0.05);治疗后第7天,观察组肺功能、血气分析、氧代谢指标的改善幅度均高于对照组(P<0.05)。结论:SIMV+PSV模式治疗COPD合并呼吸衰竭患者,在提升肺通气功能、改善机体供氧、纠正体内酸碱平衡方面的疗效均优于BiPAP模式。
Objective: To study the therapeutic effect of synchronized intermittent mandatory ventilation + pressure support ventilation (SIMV + PSV) on patients with chronic obstructive pulmonary disease (COPD) complicated with respiratory failure. Methods: Eighty COPD patients with respiratory failure were randomly divided into observation group and control group, 40 cases in each group. The control group was treated with BiPAP mode. The observation group was treated with SIMV + PSV mode , Detected and compared two groups of patients before treatment and on the seventh day after treatment, lung function, blood gas analysis, oxygen metabolism indicators of the three groups. Results: There was no significant difference in pulmonary function, blood gas analysis and oxygen metabolism index between the two groups before treatment (P> 0.05). On the 7th day after treatment, the lung function, blood gas analysis and oxygen metabolism indexes in the observation group were both improved In the control group (P <0.05). Conclusions: SIMV + PSV is superior to BiPAP in the treatment of patients with COPD and respiratory failure. It is superior to BiPAP in improving pulmonary ventilation, improving body oxygen supply and correcting acid-base balance in vivo.
