目的采用经食管实时三维超声心动图(real time three dimemensional transesophageal echocardiograph y,RT-3D-TEE)技术测量轻度主动脉瓣反流(aortic regurgitation,AR)患者主动脉窦部结构参数,探讨其在轻度AR机制中的作用。方法对76例轻度AR患者和32例无AR的对照组行RT-3D-TEE检查,测量左、右、无冠窦面积(Al、Ar、An),冠窦顶到窦部的高度(Hls、Hrs、Hns)及冠窦容积(Vl、Vr、Vn),并纳入Logistic回归模型,对进入Logistic回归的参数绘制ROC曲线。结果轻度AR组Al、Ar、An、Hls、Vl、Vr和Vn的增大是不均衡的。Hls、Sr和Vl进入Logistic同归方程(P=0.048、P=0.01 2、P=0.040),在ROC曲线下面积均>50%。结论冠窦容积的不均衡改变使主动脉窦空间结构失常,冠窦面积和窦顶高度亦发生不均衡的改变,导致轻度AR的发生。
Objective To investigate the aortic sinus structural parameters in patients with mild aortic regurgitation (AR) by real-time three-dimensional transesophageal echocardiograph y (RT-3D-TEE) The role of mild AR mechanism. Methods Twenty-six patients with mild AR and 32 patients without AR were examined by RT-3D-TEE. Left and right coronal sinus areas (Al, Ar, An), coronal sinus height ( Hls, Hrs, Hns) and the volume of the coronary sinus (Vl, Vr, Vn). Logistic regression models were also included to draw the ROC curve for the parameters that entered the Logistic regression. Results The increase of Al, Ar, An, Hls, Vl, Vr and Vn in mild AR group was not balanced. Hls, Sr and Vl entered the Logistic regression equation (P = 0.048, P = 0.01 2, P = 0.040), and the area under the ROC curve was> 50%. Conclusions The unbalanced changes in the volume of the coronary sinus make the aortic sinus space structure abnormalities. The area of the coronary sinus and the height of the sinus are also unevenly changed, resulting in the occurrence of mild AR.
