目的通过“基础胰岛素临床规范使用优化管理项目”(简称“血糖管理三人行”项目)对初始基础胰岛素治疗患者展开系统化管理,研究其在临床应用中的效果。方法选自荷泽市立医院首次使用基础胰岛素治疗的糖尿病患者150例,将其随机分为2组各75例:对照组由教育护士对患者进行常规教育,观察组为通过三人行项目进行血糖管理。观察2组管理前后血糖指标及自我管理量表评分,比较2组不良事件发生率。结果出院管理3个月后,2组患者血糖指标均较入院时下降,且观察组低于对照组;2组自我管理量表评分均升高,且观察组各项得分明显高于对照组;患者不良事件发生率明显低于对照组,差异均有统计学意义(P<0.05)。结论 “三人行项目”用于初始基础胰岛素治疗患者血糖管理可明显改善糖尿病患者的血糖及糖化血红蛋白达标率,不良事件少,具有临床应用价值。
Objective To systematically manage patients with initial basal insulin by means of Optimized Management Project for Clinical Norms of Essential Insulin (“Blood Glucose Management Threesomes”), and to study its effect in clinical application. Methods 150 cases of diabetic patients who were treated with basal insulin at Heze Municipal Hospital for the first time were randomly divided into 75 cases in each group. The control group received regular education from nurses on education. The observation group was blood glucose management through threesome project . Two groups were observed before and after the management of blood glucose and self-management scale scores, the incidence of adverse events in two groups were compared. Results After three months’ discharge management, the blood glucose of two groups were lower than that of admission, and the observation group was lower than that of the control group. The self-management scale scores of two groups increased, and the scores of the observation group were significantly higher than those of the control group. The incidence of adverse events in patients was significantly lower than that in the control group, with significant differences (P <0.05). Conclusion “Threesome project ” for the initial basal insulin therapy in patients with glycemic management can significantly improve the blood glucose and glycosylated hemoglobin compliance rate, with fewer adverse events, with clinical value.
