[目的]评价国产多西紫杉醇(艾素)联合DDP和5-Fu治疗进展期胃癌的临床疗效和安全性,并以进口多西紫杉醇(泰素帝)联合DDP和5-Fu方案作为对照。[方法]62例进展期胃癌患者随机分为艾素组(32例)与泰素帝组(30例),两组给药剂量与方法相同,分别为艾素或泰素帝75mg/m2,d1;DDP 25mg/m2,d1~3;5-Fu 200mg/m2,civ24h,d1~5,3周为1个周期。分别评价其有效率(RR)、中位疾病进展时间(TTP)、1、2年生存率与毒副反应。[结果]61例可评价疗效,艾素组与泰素帝组RR分别为46.9%与51.7%(χ2=0.143,P=0.705),两组TTP分别为5.3个月与5.5个月(χ2=1.320,P=0.100),两组1年生存率分别为50.0%和55.2%,2年生存率分别为23.3%与27.6%(χ2=0.163,P=0.686),两组间均无统计学差异。两组主要毒副发应为白细胞减少、中性粒细胞减少、粒细胞减少性发热、贫血、脱发、恶心呕吐、骨关节肌肉酸痛和乏力,艾素组的骨关节肌肉酸痛发生率较泰素帝组低,其他Ⅲ~Ⅳ度毒副反应发生率没有统计学差异。[结论]艾素联合顺铂、5-Fu方案治疗晚期疗效与泰素帝联合顺铂、5-Fu方案相当,毒副反应可以耐受。
[Objective] To evaluate the clinical efficacy and safety of domestic docetaxel combined with DDP and 5-Fu in the treatment of advanced gastric cancer and to compare the efficacy of docetaxel (Taxotere) with DDP and 5-Fu. [Methods] Sixty-two patients with advanced gastric cancer were randomly divided into two groups: Alzheimer’s group (32 cases) and Taxol group (30 cases). The two groups were given the same dose and method of Alzheimer’s or Taxol 75mg / m2, d1; DDP 25mg / m2, d1 ~ 3; 5-Fu 200mg / m2, civ24h, d1 ~ 5,3 weeks for a cycle. Respectively evaluate the efficiency (RR), the median disease progression time (TTP), 1, 2 year survival rate and toxicity. [Results] The 61 patients were evaluated for efficacy. The RRs of the group receiving Isotinol and Taxol were 46.9% and 51.7%, respectively (χ2 = 0.143, P = 0.705). The TTP of the two groups were 5.3 and 5.5 months, respectively 1.320, P = 0.100). The two-year 1-year survival rates were 50.0% and 55.2%, respectively, and the 2-year survival rates were 23.3% and 27.6% respectively (χ2 = 0.163, P = 0.686) . The two main toxicities should be leukopenia, neutropenia, neutropenia, anemia, alopecia, nausea and vomiting, joint pain and muscle soreness and osteoarthritis, Emperor group is low, the incidence of other Ⅲ ~ Ⅳ degrees of toxicity was not statistically different. [Conclusion] The combination of cisplatin and 5-Fu with etoposide and 5-Fu regimen is comparable to Taxus combined with cisplatin and 5-Fu, and the side effects can be tolerated.
