目的调查小榄地区生殖器人乳头状瘤病毒(HPV)感染情况及其基因亚型分析,为HPV感染分子流行病学研究和特异性疫苗的研制提供依据。方法采用凯普导流杂交HPV DNA检测法,对小榄地区疑似HPV感染的2 160例生殖器分泌物进行21种HPV亚型检测。结果 382例男性标本中有142例HPV检测阳性,感染率为37.17%。其中高危型占33.59%,低危型占57.25%,1 778例女性标本中有449例HPV检测阳性,感染率为25.25%,其中高危型占81.51%。女性HPV高危型感染率随着年龄的增长而升高(小于21岁除外)。男性HPV感染的高峰年龄为21~40岁,占90.14%,在31~40岁年龄段,HPV高危型感染率和低危型感染率差异无统计学意义(P>0.05)。男性和女性的生殖器HPV高危亚型检出频率较高的前五位基因型别相同,除HPVl6型之外,其他4个型别52、58、31和33占高危型的68.04%(281/413)。结论小榄地区生殖器人乳头状瘤病毒感染较严重,男性和女性HPV感染的主要高危亚型都是16,52、58、31和33,这对小榄地区HPV分子流行病学的研究和相关肿瘤的防治具有重要意义。
Objective To investigate genital HPV infection and its genotypes in Siu Lam area and provide basis for molecular epidemiology and development of HPV vaccine. Methods Kaplan-Meier flow-through hybridization HPV DNA test was used to detect 21 HPV genotypes in 2160 cases of genital secretions suspected of HPV infection in Siu Lam area. Results Of the 382 male specimens, 142 had positive HPV testing, with an infection rate of 37.17%. Among them, high-risk accounted for 33.59%, low-risk accounted for 57.25%, and HPV samples were positive for 449 of 1778 female samples, with infection rate of 25.25%, of which 81.51% were high risk type. The prevalence of high-risk HPV in women increases with age (except those younger than 21 years). The peak age of male HPV infection was 21-40 years old, accounting for 90.14%. There was no significant difference in HPV infection rate and low-risk infection rate between 31-40 years old (P> 0.05). The genotypes of HPV genotypes with high risk of genital HPV infection in both men and women were the same. Among HPV genotypes 6, 52, 58, 31 and 33, 68.04% (281 / 413). Conclusions Genital HPV infection is more serious in Siu Lam area. The main high risk subtypes of HPV infection in both men and women are 16, 52, 58, 31 and 33, which are related to HPV molecular epidemiology and related Prevention and treatment of tumors is of great significance.
