目的探讨改良WCUMS评估预测神经内科患者发生压疮的准确性,并分析相关危险因素对压疮发生的影响。方法收集120例神经科内压疮患者的临床资料,应用改良WCUMS评估表进行评分,选取性别、年龄、吸烟史、活动方式、意识状态、血清ALB、血糖、体位等12个相关因素,分析该评估表对预测压疮的准确性及相关影响因素。结果 120例患者中,WCUMS评分为极高危46例(38.33%),高危37例(30.83%),中低危3l例(23.33%),低危9例(7.50%);单因素分析显示,血清ALB、Hb、意识状态、皮肤类型、体位和活动方式是发生压疮的危险因素(P均<0.05);多因素logistic回归分析显示,血清ALB、Hb、意识状态、活动方式与压疮发生有回归关系(P<0.05);相关性分析提示,血清ALB、Hb水平与WCUMS评分成显著正相关(P均<0.05),意识状态、活动方式、压疮严重程度与WCUMS评分成显著负相关(P均<0.05)。结论应用改良的WCUMS评估表能准确地预测神经内科患者发生压疮的危险性,血清ALB、Hb水平、意识障碍及活动方式是神经内科患者发生压疮的独立危险因素。
Objective To investigate the accuracy of modified WCUMS in predicting pressure ulcer in neurology patients and analyze the influence of related risk factors on pressure ulcer. Methods The clinical data of 120 patients with neurological pressure ulcer were collected and scored by improved WCUMS assessment table. 12 related factors including gender, age, smoking history, activity pattern, state of consciousness, serum ALB, blood glucose and body position were analyzed and analyzed Evaluation table to predict the accuracy of pressure ulcers and related factors. Results Among the 120 patients, the WCUMS score was extremely high risk in 46 cases (38.33%), high risk in 37 cases (30.83%), moderate and low risk in 3l cases (23.33%) and low risk in 9 cases (7.50% Serum ALB, Hb, status of consciousness, skin type, posture and activity were the risk factors of pressure ulcer occurrence (all P <0.05). Multivariate logistic regression analysis showed that serum ALB, Hb, consciousness status, (P <0.05). Correlation analysis indicated that there was a significant positive correlation between serum ALB and Hb levels and WCUMS score (all P <0.05). There was a significant negative correlation between the state of consciousness, activity pattern, severity of pressure ulcer and WCUMS score (P <0.05). Conclusions The improved WCUMS assessment table can accurately predict the risk of pressure ulcer in patients with neurosurgical diseases. Serum ALB, Hb levels, disturbance of consciousness and activity patterns are independent risk factors of pressure ulcer in neurology patients.
