[目的]探讨依达拉奉对高血压脑出血患者血浆内皮素(ET)、降钙素基因相关肽(CGRP)的影响及对高血压脑出血患者的临床疗效分析。[方法]将采用微创手术治疗的高血压脑出血患者80例,随机分为依达拉奉治疗组和常规治疗组(各40例);依达拉奉治疗组在常规治疗的基础上用依达拉奉30mg加生理盐水100ml静脉滴注,30min内滴完,每日2次,14d为1个疗程。治疗组及常规组在1个月及3个月进行KPS评分;放射免疫法(0d、1d、3d、5d)分别测血浆ET、CGRP值,以观察依达拉奉对高血压脑出血病人血浆ET、CGRP值的影响。另取40份健康献血员血浆标本作正常对照组。[结果]①治疗组与常规组在1个月及3个月KPS评分均存在统计学差异(P﹤0.05),治疗组明显高于常规组。②入院时(0d)依达拉奉治疗组与常规组ET、CGRP值比较,P﹥0.05,差异无统计学意义。③(1d、3d、5d)中依达拉奉治疗组与常规组ET、CGRP值比较、常规组与正常对照组比较,P均﹤0.01,差异有统计学意义。[结论]依达拉奉通过降低血浆ET含量,提高CGRP水平,对脑损伤有一定保护作用,对高血压脑出血继发性脑损伤的治疗有一定的疗效并可能改善其预后。
[Objective] To investigate the effect of edaravone on plasma endothelin (ET) and calcitonin gene related peptide (CGRP) in patients with hypertensive intracerebral hemorrhage and its clinical efficacy in patients with hypertensive intracerebral hemorrhage. [Methods] 80 patients with hypertensive intracerebral hemorrhage treated with minimally invasive surgery were randomly divided into edaravone treatment group and conventional treatment group (40 cases each). Edaravone treatment group was used on the basis of routine treatment Edaravone 30mg plus saline 100ml intravenous drip, 30min drip finished, 2 times a day, 14d for a course of treatment. The KPS scores of the treatment group and the conventional group were measured at 1 month and 3 months respectively. The levels of plasma ET and CGRP were measured by radioimmunoassay (0d, 1d, 3d, 5d) to observe the effect of edaravone on the plasma of patients with hypertensive intracerebral hemorrhage ET, CGRP values. Another 40 healthy blood donors plasma samples for the normal control group. [Results] ① The KPS scores of the treatment group and the routine group at 1 month and 3 months were significantly different (P <0.05), and the treatment group was significantly higher than the conventional group. ② At admission (0d), there was no significant difference between ET and CGRP in edaravone group and conventional group (P> 0.05). (1d, 3d, 5d), edaravone treatment group compared with the conventional group ET, CGRP values, routine group and the normal control group, P <0.01, the difference was statistically significant. [Conclusion] Edaravone may have a certain protective effect on brain injury by decreasing plasma ET level and increasing CGRP level. It has a certain curative effect on the treatment of hypertensive intracerebral hemorrhage secondary brain injury and may improve its prognosis.
