目的探讨经闭孔无张力尿道中段悬吊术(transobturator vaginal tape inside-out,TVT-O)治疗压力性尿失禁(SUI)临床疗效。方法回顾性分析我院2010年5月—2012年6月对45例(年龄45~72岁,平均56岁)压力性尿失禁患者行经闭孔无张力尿道中段悬吊术。结果 45例手术顺利,未出现膀胱损伤或穿孔、血管、神经损伤,无血肿、尿瘘,无网带外露。1例术后症状明显缓解,强烈咳嗽及大笑时有少许尿液溢出。术后出现排尿困难1例,给予行网带松解术。45例随访24个月,完全治愈43例,显著好转2例,完全治愈率95.5%(43/45)。结论 TVT-O治疗压力性尿失禁操作简便,损伤小,并发症少而且安全有效。
Objective To investigate the clinical efficacy of transobturator vaginal tape inside-out (TVT-O) in the treatment of stress urinary incontinence (SUI). Methods Retrospective analysis from May 2010 to June 2012 in our hospital 45 patients (45 to 72 years of age, mean 56 years) with stress urinary incontinence underwent closed-loop tension-free urethral mid-suspension. Results Forty-five cases underwent surgery without bladder injury or perforation, blood vessels, nerve injury, hematoma, urinary fistula, and no net exposure. One patient had obvious postoperative symptom relief, and a little urine spilled when coughing and laughing loudly. Postoperative dysuria occurred in 1 case, given the line mesh with the release of surgery. Forty-five patients were followed up for 24 months, 43 cases were completely cured, 2 cases improved significantly, and the complete cure rate was 95.5% (43/45). Conclusion TVT-O treatment of stress urinary incontinence is simple, less injury, less complications and safe and effective.
