目的:探讨影响重度颅脑损伤患者ICU停留时间的相关因素,为临床防治提供依据。方法:对丽水市中心医院2009年2月-2011年10月期间收治的96例重度颅脑损伤患者病例资料进行回顾性分析,探讨影响ICU停留时间的相关因素。结果:该组96例患者中42例在ICU内停留3d,26例停留3~7d,28例停留7d。转出ICU时77例各项生命体征较平稳,顺利转出ICU,19例死亡,病死率19.79%。分析结果显示,GCS评分低者脑损伤较重,合并呼吸功能不全及血糖升高,手术时机的选择,均导致ICU停留时间明显延长,对疾病预后有不利影响,以上因素均有统计学意义。结论:重度颅脑损伤患者损伤程度、GCS评分、手术时机选择及合并症情况直接影响患者ICU停留时间。
Objective: To explore the related factors that affect the ICU stay time in patients with severe craniocerebral injury and provide the basis for clinical prevention and treatment. Methods: A retrospective analysis of 96 cases of severe craniocerebral injury patients admitted from February 2009 to October 2011 in Lishui Central Hospital was conducted to investigate the influencing factors of ICU stay time. Results: Of the 96 patients in this group, 42 patients stayed for 3 days within ICU, 26 patients stayed for 3 to 7 days and 28 patients stayed for 7 days. Out of the ICU 77 cases of various vital signs more smoothly, out of the ICU, 19 patients died, the mortality rate was 19.79%. The results showed that the lower the GCS score was, the more severe the brain damage was. The combination of respiratory insufficiency and hyperglycemia, and the timing of operation, all resulted in significantly prolonged ICU stay and adverse effect on the prognosis of the disease. The above factors were all statistically significant. Conclusion: The degree of injury, GCS score, timing of operation and complications in patients with severe craniocerebral injury have a direct impact on ICU stay time.
