目的:探讨社区全科医生团队健康管理模式对原发性高血压患者的影响。方法:选取2015年2月至2016年5月在东莞市寮步镇社区卫生服务中心门诊进行治疗的106例原发性高血压患者,依据随机抽签法均分为观察组和对照组,各53例。对照组患者进行一般性管理,观察组患者进行社区全科医生团队综合干预管理,对比两组患者血压情况、管理服务满意度以及依从性。结果:两组患者管理前收缩压以及舒张压比较,差异均无统计学意义(P>0.05)。管理前后两组患者的收缩压以及舒张压比较,差异具有统计学意义(P<0.05),且管理后观察组患者的收缩压以及舒张压下降程度明显高于对照组,组间比较,差异具有统计学意义(P<0.05)。观察组管理服务满意度为98.11%,明显高于对照组的84.91%,差异具有统计学意义(P<0.05)。观察组患者的按时服药率、体质量控制率以及饮食控制率均高于对照组,差异具有统计学意义(P<0.05)。结论:社区全科医生团队健康管理模式有效降低原发性高血压患者的血压,提高管理服务满意度,也提高患者的依从性。
Objective: To investigate the impact of community health team management on patients with essential hypertension. Methods: From February 2015 to May 2016, 106 patients with essential hypertension who were treated in community clinics of Liaobu Community Health Center in Dongguan City were randomly divided into observation group and control group with 53 example. Patients in the control group were managed in a general manner. Patients in the observation group were managed by the community general practitioner team for comprehensive intervention. Blood pressure, management service satisfaction and compliance were compared between the two groups. Results: There was no significant difference in systolic blood pressure and diastolic blood pressure between the two groups before management (P> 0.05). The systolic blood pressure and diastolic blood pressure in two groups before and after management were significantly different (P <0.05), and the systolic blood pressure and diastolic blood pressure in the observation group were significantly higher than those in the control group Statistical significance (P <0.05). The satisfaction rate of the observation group management service was 98.11%, which was significantly higher than that of the control group (84.91%), the difference was statistically significant (P <0.05). The patients in the observation group took drugs on time, the body weight control rate and the diet control rate were higher than the control group, the difference was statistically significant (P <0.05). CONCLUSIONS: Community GPs’ team health management models effectively reduce blood pressure in patients with essential hypertension, improve management service satisfaction, and improve patient compliance.
