目的比较咳嗽咳痰长于2 w与短于2 w肺结核可疑症状者肺结核发生率,为更准确定义肺结核可疑症状提供科学依据。方法对2010—2015年来医院第一门诊部就诊的2 487例咳嗽咳痰长于2 w(A组)与1 151例短于2周(B组)的肺结核可疑症状者的临床诊断资料进行回顾性分析,分别按少年(<15岁)、青年(15~44岁)、中年(45~64岁)、老年(≥65岁)年龄别和春(3~5月)、夏(6~8月)、秋(9~11月)、冬(12月、1~2)就诊时间段应用2检验,比较A、B两组间肺结核发生率的差异。结果 A、B组患者肺结核的发生率分别为60.96%(1 516/2 487)、61.34%(706/1 151),其中:A、B组中少年、青年、中年、老年患者肺结核的发生率分别为36.36%(4/11)、70.48%(542/769)、61.47%(654/1 064)、49.14%(316/643)和0(0/3)、66.58%(267/401)、62.55%(304/486)、51.72%(135/261);A、B组患者在春、夏、秋、冬季节肺结核的发生率分别为61.95%(451/728)、62.81%(353/562)、58.31%(249/427)、55.92%(463/828)和64.78%(195/301)、60.84%(160/263)、55.69%(137/246)、59.28%(214/361)。经2检验比较,各年龄段以及不同季节中A、B两组患者肺结核的发生率均无显著性差异。结论目前肺结核可疑症状定义中的“咳嗽、咳痰≥2 w”可适宜缩短,以利用肺结核病例的及时发现和尽早治疗。
Objective To compare the incidence of tuberculosis among those with cough and expectoration of more than 2 weeks and less than 2 weeks of suspicious symptoms of tuberculosis, so as to provide a scientific basis for more accurate definition of suspicious symptoms of tuberculosis. Methods The clinical diagnostic data of 2 487 patients with cough and expectoration longer than 2 weeks (group A) and 1 151 cases of tuberculosis suspicious symptoms shorter than 2 weeks (group B) from the first clinic of the hospital from 2010 to 2015 were retrospectively reviewed Analysis showed that there were no significant differences between the two groups according to age (<15 years old), youth (15-44 years old), middle age (45-64 years old), aged (≥65 years old) Month, autumn (September to November), winter (December, 1 to 2) treatment time period application 2 test, A, B between the two groups compared the incidence of tuberculosis. Results The incidence of pulmonary tuberculosis in group A and group B was 60.96% (1 516/2 487) and 61.34% (706/1 151), respectively. After the 2 test comparison, there was no significant difference in the incidence of tuberculosis between the two groups in all age groups and in different seasons. Conclusions At present, the definition of “suspicious symptoms of tuberculosis”, “cough, sputum ≥ 2 w” may be appropriate to shorten the use of tuberculosis cases in time to detect and treat as soon as possible.
