目的:探讨院外急救中改良早期预警评分(MEWS)系统在急诊内科中的应用价值。方法临床纳入急诊内科就诊的内科疾病患者90例,根据患者在院外急救中有无进行MEWS系统评分分为研究组与对照组。研究组院外急救中进行MEWS系统评分,对照组院外急救中未进行MEWS系统评分,观察两组患者ICU入住率以及死亡率;另外,分析MEWS系统评分与ICU入住率的相关性。结果研究组ICU入住率、意外事件发生率及死亡率明显低于对照组(P<0.05);相关性分析显示,不同MEWS系统评分与ICU入住率存在正相关,与疾病的预后存在负相关(P <0.05);研究组抢救成功率、患者满意度较对照组明显提高(P<0.05);患者的出诊反应时间、住院时间明显短于对照组(P<0.05)。结论MEWS系统评分对预测急诊内科患者就诊时的情况以及预后均较为准确,应该作为内科急诊患者入院评估病情的首选。“,”Objective To assess the application of a modified early warning score (MEWS) system for prehospital rescue in department of emergency internal medicine. Methods A total of 90 patients with internal diseases were enrolled into the current study. They were divided into two groups: a research group which was assessed using MEWS during prehospital rescue and a control group. Both groups were compared for intensive care unit (ICU) admission rate and mortality to analyze their relationship. Results The research group produced a markedly lower ICU admission rate, less unexpected events and mortality than the control (P<0.05). According to correlation analysis, MEWS scores were positively related with ICU admission rate, but negatively related with prognosis (P<0.05). Compared with the control, remarkably improvement in successful rescue rate and patient satisfaction was seen in the research group (P<0.05), with shorter diagnosis time and duration of hospitalization (P<0.05). Conclusion Using the MEWS system, patients in department of emergency internal medicine can be better evaluated for their condition and prognosis. This system can be adopted as the first choice to assess patients before admission to department of emergency internal medicine.
