目的 :了解未经抗病毒治疗的慢性乙型肝炎患者乙型肝炎病毒基因 YMDD变异及其与前 C区变异、HBV基因型的相关性 ,HBV DNA水平与变异发生的关系。方法 :从 1 2 6例均未接受拉米夫定治疗和近一年内未经任何抗病毒治疗的慢性乙肝患者随机抽取 2 5例为研究对象。血清 YMDD变异检测运用微板核酸杂交 -核酸定量法 ,HBV基因型采用 PCR微板核酸杂交 - EL ISA技术 ,HBV DNA定量采用荧光定量 PCR分析系统 ,HBV- M采用 EL ISA法。结果 :7例存在 YMDD变异 ,变异者 HBe Ag全部阳性 ,不同的 HBV基因型形式存在不同的 YMDD变异率 ,HBV DNA水平与 YMDD变异的发生不呈正相关结论 :HBV存在 YMDD野生变异株 ,并非完全由抗病毒治疗引起 ,变异株与前 C区变异和 HBV基因型的相关性以及 HBVDNA水平和变异发生的关系有待进一步研究
Objective: To investigate the relationship between hepatitis B virus (HBV) YMDD mutation and its precore mutation and HBV genotype, HBV DNA level and mutation in patients with chronic hepatitis B without antiviral therapy. Methods: A total of 25 patients with chronic hepatitis B who did not receive lamivudine and did not have any antiviral treatment within a year were randomly selected from 126 patients. Serum YMDD mutation detection using microplate nucleic acid hybridization - nucleic acid quantification, HBV genotype by PCR microplate nucleic acid hybridization - EL ISA technology, quantitative determination of HBV DNA using quantitative PCR analysis system, HBV-M by EL ISA method. Results: YMDD mutation was found in 7 cases, and HBeAg was all positive in the mutation. Different genotypes of HBV had different YMDD mutation rates. HBV DNA level did not correlate with YMDD mutation. Conclusion: The presence of wild-type YMDD variant in HBV is not complete The relationship between the variants and pre-C region mutations and HBV genotypes, as well as the relationship between HBVDNA levels and mutation occurrence, caused by antiviral therapy remains to be further studied
