目的探讨维吾尔族肥胖(OB)伴胰岛素抵抗(IR)患者血清内脂素的变化及意义。方法根据胰岛素抵抗诊断标准(胰岛素抵抗指数≥2.8),将患者分为肥胖伴胰岛素抵抗组(OB伴IR组)31例,肥胖无胰岛素抵抗组(OB无IR组)26例,另选14例健康体检者为正常对照组。半定量RT-PCR方法检查研究对象网膜和皮下脂肪组织内脂素mRNA的表达水平。对所有受试者采用稳态模式评估法(HOMA)评价胰岛素抵抗(IR),采用ELISA法测定血清内脂素水平,同时进行临床生化参数的检测。结果(1)OB伴IR组血清内脂素水平高于OB无IR组,差异有统计学意义(P<0.05)。(2)血清内脂素与体质量、甘油三酯(TG)呈正相关,与高密度脂蛋白-胆固醇(HDL-C)呈负相关。(3)3组网膜与皮下脂肪组织内脂素mRNA表达水平差异无统计学意义。结论血清内脂素水平的升高可能与维吾尔族肥胖患者的胰岛素抵抗有关。
Objective To investigate the changes and significance of serum visfatin in Uighur obese patients with insulin resistance (IR). Methods According to the diagnostic criteria of insulin resistance (insulin resistance index ≥2.8), the patients were divided into obesity with insulin resistance group (OB with IR group) 31 cases, obesity without insulin resistance group (OB without IR group) 26 cases, another 14 cases Health examination for the normal control group. Semiquantitative RT-PCR was used to examine the expression of adiponectin mRNA in the omental and subcutaneous adipose tissue of the study subjects. All subjects were evaluated for insulin resistance (IR) using the steady-state model assessment (HOMA), serum visfatin levels were measured by ELISA, and clinical biochemical parameters were measured. Results (1) The level of serum visfatin in OB with IR group was higher than that in OB without IR group, the difference was statistically significant (P <0.05). (2) Serum visfatin was positively correlated with body weight, triglyceride (TG) and negatively correlated with high density lipoprotein-cholesterol (HDL-C). (3) The expression of visfatin mRNA in the omentum and subcutaneous adipose tissue of 3 groups had no statistical significance. Conclusions Elevated serum visfatin levels may be related to insulin resistance in obese Uigur patients.
