目的探讨新诊断酮症倾向T2DM患者肌酶谱和肌红蛋白(Mb)水平的变化。方法选取新诊断酮症倾向T2DM患者(K-T2DM组)97例、非酮症倾向T2DM患者(T2DM组)49例和健康对照者(NC组)48名,检测各组肌酶谱、Mb及其他生化指标,分析肌酶谱和Mb的影响因素。结果 K-T2DM组较NC组肌酸激酶(CK)、肌酸激酶同工酶(CK-OH)、乳酸脱氢酶(LDH)、天门冬氨酸氨基转移酶(AST)和Mb水平高(P<0.05),较T2DM组CK-OH、LDH和Mb水平高(P<0.05或P<0.01)。多元逐步回归分析显示,HbA1c是影响CK、Mb的主要因素(r2=0.052、0.051,P<0.01),FPG是影响LDH的主要因素(r2=0.043,P<0.01),BMI、HbA1c是影响CK-OH的主要因素(r2=0.040、0.025,P<0.05)。结论酮症倾向T2DM患者肌酶谱和Mb水平存在异常变化。
Objective To investigate the changes of muscle enzymes and myoglobin (Mb) in newly diagnosed ketosis-prone T2DM patients. Methods Ninety-seven patients with newly diagnosed ketosis-prone T2DM (K-T2DM group), 49 non-ketotic T2DM patients (T2DM group) and 48 healthy controls (NC group) were enrolled in this study. Muscle enzymes, Mb and other Biochemical indicators, analysis of muscle enzymes and Mb influence factors. Results The levels of creatine kinase (CK), creatine kinase (CK-OH), lactate dehydrogenase (LDH), aspartate aminotransferase (AST) and Mb in K- P <0.05). Compared with T2DM group, CK-OH, LDH and Mb levels were higher (P <0.05 or P <0.01). Multivariate stepwise regression analysis showed that HbA1c was the main factor affecting CK and Mb (r2 = 0.052,0.051, P <0.01). FPG was the main factor affecting LDH (r2 = 0.043, P <0.01) -OH of the main factors (r2 = 0.040, 0.025, P <0.05). Conclusion There are abnormal changes in muscle enzymes and Mb levels in ketosis-prone T2DM patients.
