目的 选择 2 0例心脏病患儿 ,观察在心脏不停搏心内直视手术麻醉、灌注期间心律失常的性质和种类。方法 采用大剂量芬太尼 +静吸复合麻醉 ,高流量体外转流。观察麻醉前、麻醉中、转流手术操作期间、开放腔静脉、返回病房时心律失常变化。结果 在麻醉平稳、血气分析及电解质正常情况下 ,由于手术操作等因素可引起多种心律失常 ,其中ST段心肌缺血性改变具有临床意义。结论 小儿心脏不停搏心内直视手术由于手术操作刺激及灌注压偏低可引起心肌缺血性改变。
Objective To select 20 children with heart disease and observe the anesthesia in open heart surgery and the nature and type of arrhythmia during perfusion. Methods high-dose fentanyl + static inhalation anesthesia, high-flow in vitro bypass. Observed before anesthesia, anesthesia, during the operation of bypass surgery, open vena cava, return to the ward arrhythmia changes. Results In a stable anesthesia, blood gas analysis and electrolyte under normal circumstances, due to surgical operation and other factors can cause a variety of arrhythmia, ST segment myocardial ischemic changes of clinical significance. Conclusion Myocardial ischemic changes can be induced by open heart surgery in children with open heart surgery due to low surgical pressure and low perfusion pressure.
