用放免法检测61例肝硬化患者与正常对照组的血浆内皮素-1(ET-1)和肿瘤坏死因子-α(TNF-α)。结果:肝硬化组血浆ET-1及TNF-α水平显著高于对照组(P均<0.001),且在肝功能Child-Pugh分级中,呈现C>B>A级的规律。组间分析表明肝硬化患者食管静脉曲张伴大中量腹水组(LC4)及仅有大中量腹水组(LC3)的血浆ET-1和TNF-α均显著高于食管静脉曲张伴少量腹水或无腹水组(LC2),LC2组高于无腹水和亦无静脉曲张组(LC1)(P均<0.05)。LC1组及Child-PughA级患者与对照组间无明显差别(P>0.05)。相关分析显示肝硬化者血浆ET-1和TNF-α间存在明显正相关。认为肝硬化者血浆ET-1和TNF-α水平可反映肝硬化的严重程度,并直接或间接的在门脉高压的形成发展中可能起重要作用
The plasma levels of ET-1 and TNF-α in 61 cirrhotic patients and normal controls were detected by radioimmunoassay. Results: The levels of plasma ET-1 and TNF-α in cirrhosis group were significantly higher than those in control group (all P <0.001). And in Child-Pugh classification of liver function, C> B> A grade was found. Intergroup analysis showed that the levels of plasma ET-1 and TNF-α in patients with cirrhosis esophageal varices with large amount of ascites group (LC4) and only large amount of ascites group (LC3) were significantly higher than esophageal varices with a small amount of ascites No ascites group (LC2), LC2 group than no ascites and no varicose vein group (LC1) (P all <0.05). There was no significant difference between LC1 group and Child-PughA group and control group (P> 0.05). Correlation analysis showed that there was a significant positive correlation between plasma ET-1 and TNF-α in cirrhotic patients. The level of plasma ET-1 and TNF-α in cirrhotic patients may reflect the severity of cirrhosis and may play an important role in the development of portal hypertension, either directly or indirectly
