高剂量(达10Gy)全身照射(TBI)越来越多地应用于恶性肿瘤和血液病的治疗。本文着重对骨髓移植(BMT)前高剂量TBI诸问题进行论述。BMT前TBI的主要作用(1)免疫抑制——这是移植物生长的必要条件,特别是对再障患者。移植前采用TBI可以减少或防止移植的排斥。(2)杀灭体内恶性瘤细胞——采用TBI,其杀灭全身恶性细胞比化疗更为有效。(3)骨髓清除——通过TBI清除全部造血细胞成分,使骨髓腔变成一个“空腔”,此时被移植的骨髓才能存活。
High-dose (up to 10 Gy) total body irradiation (TBI) is increasingly used for the treatment of malignant tumors and hematological diseases. This article focuses on the issues of high-dose TBI before bone marrow transplantation (BMT). The main role of TBI before BMT (1) Immunosuppression - This is a necessary condition for graft growth, especially for patients with aplastic anemia. Using TBI before transplantation can reduce or prevent the rejection of the transplant. (2) killing malignant tumor cells in the body - using TBI, which kills systemic malignant cells is more effective than chemotherapy. (3) Bone Marrow Debridement - The TBI removes all hematopoietic cellular components, making the marrow cavity a “cavity,” when the transplanted bone marrow can survive.
