目的:系统评价发酵乳联合常规治疗方案清除幽门螺杆菌(Helicobacter pylori,H.pylori)感染的临床疗效。方法:通过检索Pubmed,Embase,中国知网,中国生物医学文献数据库,万方知网和维普等数据库,收集各数据库建库至2015年2月间与发酵乳辅助治疗H.pylori感染相关研究的随机试验或半随机对照试验,分析根除率与不良反应发生率的合并RR值,并进行亚组、敏感性分析和发表偏倚检测。结果:共纳入9项(1 644例)研究,发酵乳联合常规根除疗法与单独常规根除疗法H.pylori根除率分别为79.5%和67.0%,其合并RR为1.186(95%CI:1.118~1.257)。总不良反应发生率的合并RR为0.706(95%CI:0.373~1.340)。结论:益生菌可提高常规疗法对H.pylori的根除率,但不能有效降低不良反应的发生风险。
Objective: To systematically evaluate the clinical efficacy of fermented milk combined with routine treatment in the treatment of Helicobacter pylori (H.pylori) infection. Methods: The databases of Pubmed, Embase, CNKI, China Biomedical Literature Database, Wanfang Zhifu and Vipu were searched to collect the data from the database construction to February 2015, which is related to fermented milk adjuvant therapy for H.pylori infection Randomized trials or semi-randomized controlled trials were performed to analyze the combined RR of eradication rates and incidence of adverse events and to perform subgroups, sensitivity analyzes, and publication bias tests. Results: A total of nine (1664) studies were included. The eradication rates of H.pylori by fermented milk combined with conventional eradication therapy and conventional eradication therapy were 79.5% and 67.0%, respectively, and the combined RR was 1.186 (95% CI: 1.118-1.257 ). The combined RR for overall adverse events was 0.706 (95% CI: 0.373 to 1.340). Conclusion: Probiotics can improve the eradication rate of H.pylori in conventional therapy, but can not effectively reduce the risk of adverse reactions.
