目的 结合应用颈阔肌肌蒂皮瓣修复口腔癌术后组织缺损的体会，对颈阔肌肌蒂皮瓣的设计、切取及适应证等有关问题进行论述。方法 颈阔肌肌蒂皮瓣与颈淋巴清扫术的切口同时设计，根据缺损部位和大小设计肌蒂皮瓣，待口腔癌联合根治术完成，将其旋转由颌下隧道转移至口腔不同部位，修复舌、口底７ 例，颊部４ 例，牙龈口底１ 例。结果 本组１２ 例中１１ 例颈阔肌肌蒂皮瓣全部成活。经６ 个月～５ 年随访，外形、功能满意。结论 颈阔肌肌蒂皮瓣具有应用方便、血运好、成活力强，可在颈淋巴清扫同一术野内完成，操作技术容易等优点，是同期修复肿瘤术后口腔小型缺损的一种方便和快捷的手段。
OBJECTIVE: To discuss the related issues about the design, removal and indications of platysma flap pedicled with platysma flaps for the repair of postoperative tissue defects of oral cancer. Methods The flap of the platysma muscle and the incision of neck dissection were designed simultaneously. The flap of the muscle pedicle was designed according to the site and size of the defect. When the radical operation of oral cancer was completed, the rotation was transferred from the submandibular tunnel to different parts of the oral cavity, Repair the tongue, mouth in 7 cases, 4 cases of cheek, gum bottom in 1 case. Results In this group, 11 cases of platysma flap all survived. After 6 months to 5 years follow-up, shape, functional satisfaction. Conclusion The platysma flap pedicled with platysma muscle has the advantages of convenient application, good blood supply, strong viability, complete operation in the same surgical field of cervical lymph node dissection, easy operation and so on. It is a convenient and effective method to repair small oral defects after operation Quick way.
