目的探讨h CG注射对子宫内膜厚度和体外受精/卵细胞浆内单精子注射-胚胎移植(IVF/ICSI-ET)妊娠结局的影响。方法回顾性分析2014年1月-2015年6月在广东省妇幼保健院生殖医学中心行IVF/ICSI-ET患者的临床资料及妊娠结局,将所有周期以35岁为界,分为组1(<35岁)、组2(≥35岁);并按照h CG注射日经阴道超声测量的子宫内膜厚度将每组又分为4个亚组:A组患者子宫内膜厚度<8 mm,B组8~10 mm,C组>10 mm且<14 mm,D组≥14 mm。在A1、B1、C1、D1组及A2、B2、C2、D2组中,比较在相同年龄段下子宫内膜厚度对妊娠结局的影响。结果 h CG注射日子宫内膜厚度与年龄呈负相关(P<0.05),与Gn的天数呈正相关(P<0.05),与Gn量、h CG注射日E2水平以及获卵数无相关性(P>0.05)。在相同年龄段中各亚组间的临床资料差异无统计学意义(P>0.05),随着子宫内膜厚度的增加,临床妊娠率、胚胎种植率呈上升的趋势,差异有统计学意义(P<0.05);流产率、生化妊娠率及宫外孕的发生率差异无统计学意义(P>0.05)。结论 h CG注射日子宫内膜的平均厚度呈现随着年龄增长而逐渐降低的改变,当子宫内膜厚度<8 mm时其临床妊娠率、胚胎种植率明显下降。
Objective To investigate the effect of h CG injection on endometrial thickness and IVF / ICSI-ET pregnancy outcome in IVF / ESCs. Methods The clinical data and pregnancy outcomes of patients with IVF / ICSI-ET at the Reproductive Medicine Center of Guangdong MCH from January 2014 to June 2015 were retrospectively analyzed. All the patients were divided into group 1 <35 years old) and group 2 (≥35 years old). The endometrial thickness measured by transvaginal ultrasonography on h CG injection day was divided into four subgroups: group A, endometrial thickness <8 mm, Group B 8 to 10 mm, Group C> 10 mm and <14 mm, Group D 14 mm. In groups A1, B1, C1, D1 and groups A2, B2, C2 and D2, the effect of endometrial thickness on pregnancy outcome was compared in the same age group. Results The endometrial thickness at h CG injection was negatively correlated with age (P <0.05), and was positively correlated with the days of Gn (P <0.05), but not with the Gn level, E2 level at h CG injection and the number of oocytes retrieved P> 0.05). There was no significant difference in the clinical data between subgroups in the same age group (P> 0.05). With the increase of endometrial thickness, clinical pregnancy rate and embryo implantation rate showed an upward trend with a significant difference (P> P <0.05). There was no significant difference in the rates of miscarriage, biochemical pregnancy rate and ectopic pregnancy (P> 0.05). Conclusions The average thickness of endometrium on h CG injection shows a gradually decreasing change with age. When the thickness of endometrium is less than 8 mm, the clinical pregnancy rate and embryo implantation rate obviously decrease.
