目的比较Raypex5根尖定位仪两种测量法测定牙齿根管工作长度的准确性。方法选择2009年7月至2009年11月北京口腔医院收治的慢性根尖周炎患者拔除的离体牙40颗为根尖周破坏组;以正畸拔除的单直根管且根尖孔发育完全的健康牙40颗作为根尖周完整组。采用离体牙实测法测量各离体牙根管长度的理论实际值;分别以Raypex5根尖定位仪显示屏上绿格位置测得长度(A法)和红格位置测得长度减去0.5mm(B法)作为工作长度。测量值与理论实际值的差值为IF值,比较两种测量法IF值的频数分布。结果根尖完整组中,A、B两种测量法的准确率分别是85%和87.5%,差异无统计学意义(χ2=0.11,P>0.05);根尖破坏组中,B法的准确率72.5%,高于A法的50%,差异有统计学意义(χ2=4.27,P<0.05)。A法在根尖完整组中的准确率85%,高于根尖破坏组的50%,差异有统计学意义(χ2=11.17,P<0.01);B法在根尖完整组中的准确率87.5%,高于根尖破坏组的72.5%,差异无统计学意义(χ2=2.8,P>0.05)。结论 Raypex5根尖定位仪能够准确测量根管长度,其数码条到达红格后测得的长度减去0.5mm更接近根管真实工作长度。
Objective To compare the accuracy of Raypex 5 apical positioning instrument with two measurement methods to determine the working length of dental root canal. Methods 40 cases of isolated periapical teeth from patients with chronic periapical periodontitis treated in Beijing Stomatological Hospital from July 2009 to November 2009 were periapical week destructive group. Complete healthy teeth 40 as periapical complete group. The measured actual length of each root canals was measured by using the measured teeth in vitro. The measured length of the green grid (A method) on the display of Raypex5 tip and the measured length of the red grid were subtracted by 0.5 mm (Method B) as the working length. The difference between the measured value and the theoretical actual value is the IF value, and the frequency distribution of the IF values of the two measurement methods is compared. Results In the apical complete group, the accuracy rates of A and B were 85% and 87.5% respectively, with no significant difference (χ2 = 0.11, P> 0.05). In the apical destruction group, the accuracy of the B method The rate was 72.5%, which was higher than that of A method (χ2 = 4.27, P <0.05). The accuracy of method A in root apical complete group was 85%, which was higher than that in root apical destruction group (χ2 = 11.17, P <0.01). The accuracy of method B in apical complete group 87.5%, which was higher than 72.5% of the apical disruption group (χ2 = 2.8, P> 0.05). Conclusion Raypex5 apical positioner can accurately measure the root canal length, the digital strip to reach the red grid measured minus 0.5mm closer to the actual working length of the root canal.
