目的 探讨胃癌螺旋CT (SCT)征象与MVD及VEGF、TGF β1蛋白表达间的关系。方法 对 4 0例进展期胃癌病人行SCT平扫及三期动态增强扫描。 4 0例均于检查后一周内施行全胃或部分胃切除手术、剖腹探查术。术后标本行常规石蜡包埋及 5 μm切片。然后行VEGF、MVD、TGF β1免疫组化染色。结果 ①在淋巴结转移阳、阴性组 ,VEGF的阳性表达率分别为 85 .71% (12 / 14 )和 15 .38% (4 / 2 6 ) ;在肝转移阳、阴性组 ,VEGF阳性表达率分别为 83.33% (5 / 6 )和 32 .35 % (11/ 34)。②在浆膜侵犯有、无组 ,TGF β1阳性表达率分别为 4 4 .4 4 % (12 / 2 7)和 7.6 9% (1/ 13) ;在腹腔转移阳、阴性组 ,TGF β1阳性表达率分别为 80 .0 0 % (4 / 5 )和 2 2 .86 % (8/ 35 )。③MVD值变化范围为 4 .8~ 4 9,平均值 2 6 .0 4(标准差 11.96 )。在VEGF阳、阴性组 ,MVD值分别为 30 .91± 12 .2 2和 2 2 .78± 10 .83;在TGF β1阳、阴性组 ,MVD值分别为 2 9.80± 12 .4 3和 2 4 .4 2± 11.6 0。④在 4 0例胃癌中 ,共有 9例胃癌出现VEGF与TGF β1阳性表达 (P =0 .0 0 2 ,r =0 .4 6 8)。结论 VEGF的表达与胃癌SCT征象上的淋巴结转移及肝转移有关 ;TGF β1的表达与胃癌SCT征象上的浆膜侵犯及腹腔转移有关 ;TGF β1可能通过上调VEGF?
Objective To investigate the relationship between spiral CT (SCT) signs and the expressions of MVD, VEGF and TGF β1 in gastric cancer. Methods 40 cases of advanced gastric cancer patients underwent SCT scan and three dynamic contrast-enhanced scan. All 40 patients underwent whole or partial gastrectomy and laparotomy in one week after the examination. The specimens were paraffin embedded and 5 μm sections. Then VEGF, MVD, TGFβ1 immunohistochemical staining. Results ① The positive expression rate of VEGF in positive and negative lymph node metastasis was 85.7% (12/14) and 15.38% (4/26%), respectively. The positive expression rate of VEGF in positive and negative liver metastases 83.33% (5/6) and 32.35% (11/34), respectively. (2) The positive rates of TGFβ1 expression were 44.44% (12/27) and 7.69% (1/13) in serosal invasion and no histological changes respectively. In the positive and negative groups of TGFβ1 The expression rates were 80.0% (4/5) and 22.86% (8/35) respectively. ③ The value of MVD varies from 4.8 to 4.9, with an average of 26.04 (standard deviation 11.96). The values of MVD in positive and negative VEGF groups were 30.91 ± 12.22 and 2 2 .78 ± 10.83, respectively. The positive and negative MVD values in TGFβ1 positive and negative groups were 29.80 ± 12.33 and 2 4 .4 2 ± 11.6 0. ④ In 40 cases of gastric cancer, a total of 9 gastric cancers showed positive expression of VEGF and TGFβ1 (P = 0.002, r = 0.4668). Conclusion The expression of VEGF is related to lymph node metastasis and liver metastasis in SCT sign of gastric cancer. The expression of TGF-β1 is related to serosa invasion and peritoneal metastasis in SCT sign of gastric cancer.
