目的调查围绝经期门诊女性乳腺筛查状况,探讨影响围绝经期女性乳腺癌筛查接受度的影响因素。方法选取2010年1月-2016年9月在上海交通大学附属第六人民医院围绝经期门诊就诊的1 133例患者为研究对象,通过问卷调查的方法建立围绝经期就诊随访档案,对乳腺筛查的情况进行分析。结果研究对象平均年龄(51.52±4.40)岁;就诊前1年之内行乳腺癌筛查者254例(22.42%),其中乳腺超声、乳腺钼靶、超声合并钼靶、其他检查构成比分别为66.54%、24.41%、6.30%、2.76%。就诊后从未行乳腺癌筛查者中仅73.37%接受检查建议。乳腺钼靶结果 BI-RADS分类1级占74.70%;乳腺超声检查小叶增生占77.51%。多元Logistic回归分析显示,经济收入偏低者就诊前乳腺检查接受度较低(OR=1.78,95%CI:1.32~2.38);就诊后文化程度低的患者对乳腺检查接受度仍较低(OR=1.596,95%CI:1.152~2.211)。结论围绝经期女性对乳腺检查接受度较低,其中做过自检的患者较少,且乳腺检查的接受度与文化程度、经济收入有关,因此应加强对围绝经期女性乳腺检查必要性的宣传教育。
Objective To investigate the status of breast screening in women during perimenopausal clinic and to explore the influencing factors on the acceptance of breast cancer screening in perimenopausal women. Methods From January 2010 to September 2016, 1 133 cases of perimenopausal patients attending the Sixth People’s Hospital affiliated to Shanghai Jiaotong University were enrolled in this study. The follow-up files of perimenopausal period were established by questionnaire. Check the situation for analysis. Results The average age of the subjects was (51.52 ± 4.40) years old; 254 cases (22.42%) were screened for breast cancer within one year before treatment. Among them, the ratio of breast ultrasound, mammography, mammography and ultrasound was 66.54 %, 24.41%, 6.30%, 2.76%. Only 73.37% of breast cancer screening patients who did not undergo post-treatment examination were recommended. Breast mammography results BI-RADS classification of the first grade accounted for 74.70%; lobular hyperplasia of breast ultrasound accounted for 77.51%. Multivariate Logistic regression analysis showed that patients with low income received low premarital breast examinations (OR = 1.78, 95% CI: 1.32-2.38); patients with low education received low breast examinations (OR = 1.596, 95% CI: 1.152-2.211). Conclusions Perimenopausal women have a lower acceptance rate of breast examinations. Among them, there are fewer patients who have done self-examinations, and the acceptability of breast examinations is related to their educational level and economic income. Therefore, the need for breast examinations in perimenopausal women should be strengthened Publicity and education.
