目的 探索慢性胃炎胃粘膜肠化生和不典型增生超微结构及其分子生物学的病理生理学变化。方法 对 16 8例慢性浅表性胃炎、慢性萎缩性胃炎和胃癌患者胃粘膜 ,采用组织化学染色、扫描电镜附能谱仪、透射电镜附能谱仪、图像分析仪、放射免疫法和化学发光法等现代科学技术手段 ,同步检测胃粘膜和胃粘膜上皮细胞超微结构、微量元素、DNA、cAMP和SOD ,以及3 H TdRLCT、LPO。结果 胃粘膜IM分度、分型和ATP分度 ;上皮细胞超微结构 ;胃粘膜、细胞核和线粒体的Zn、Cu、DNA、cAMP和SOD ;血清LPO和3 H TdRLCT ,在HC组、ATP 组、IM +ATP 组、IM +ATP 组、IMⅡb组和CA组的组间 ,有显著性差异 ,P <0 0 5~ 0 0 1;IM +ATP 组、IMⅡb组和CA组组间 ,无显著性差异 ,P >0 0 5。结论 ATP 、IMⅡb和CA有相似的上皮细胞超微结构与分子生物学变化 ,因而有相似的发生、发展和转归的规律 ,有助于防治的探索
Objective To explore the pathological changes of intestinal metaplasia and dysplasia in gastric mucosa of chronic gastritis and its molecular biology. Methods Gastric mucosa of 16 8 patients with chronic superficial gastritis, chronic atrophic gastritis and gastric cancer were examined by histochemical staining, scanning electron microscopy, energy dispersive spectroscopy, transmission electron microscopy, image analyzer, radioimmunoassay and chemiluminescence France and other modern scientific and technological means, synchronous detection of gastric mucosa and gastric mucosal epithelial ultrastructure, trace elements, DNA, cAMP and SOD, and 3 H TdRLCT, LPO. Results The gastric mucosal IM index, classification and ATP index; the ultrastructure of epithelial cells; Zn, Cu, DNA, cAMP and SOD in gastric mucosa, nucleus and mitochondria; LPO and 3 H TdRLCT in HC group and ATP group , IM + ATP group, IM + ATP group, IMⅡb group and CA group, P <0.05-0.01; there was no significant difference between IM + ATP group, IMⅡb group and CA group Sex differences, P> 0 0 5. Conclusions ATP, IMⅡb and CA have similar ultrastructural and molecular biological changes of epithelial cells, and thus have similar occurrence, development and prognosis rules, which are helpful to the exploration of prevention and treatment
