目的应用高频彩色多普勒超声结合组织声触诊量化技术(VTQ)分析乳腺黏液腺癌的特征。方法 21例乳腺黏液腺癌患者,均为女性,年龄48~83岁,平均年龄61.15岁。应用高频彩色多普勒超声分析其乳腺黏液腺癌的声像图特征,根据病理组织分型分为单纯型乳腺黏液腺癌和混合型乳腺黏液腺癌两组,并结合VTQ检测肿块的弹性特征[剪切波速度(Vs)]。结果乳腺黏液腺癌的形态不规则,边缘较清晰,后方回声增强,肿块内部回声低,内有微钙化者较少,肿瘤内血流分布少;单纯型和混合型乳腺黏液腺癌之间的内部回声均匀性差异有统计学意义。两组的VTQ参数差异有统计学意义[单纯型乳腺黏液腺癌组Vs=(1.15±0.75)m/s,混合型乳腺黏液腺癌组Vs=(4.32±0.63)m/s;P<0.05],单纯型乳腺黏液腺癌组肿瘤内部硬度较周边和正常乳腺组织的低,混合型乳腺黏液腺癌组硬度较周边和正常乳腺组织的高(P<0.05)。结论乳腺黏液腺癌常规声像图具有一定的特征,VTQ的应用对于乳腺黏液腺癌的诊断具有进一步的补充意义。
Objective To analyze the characteristics of breast mucinous adenocarcinoma using high-frequency color Doppler ultrasound combined with tissue acoustic palpation quantification (VTQ). Methods Twenty-one patients with breast mucinous adenocarcinoma were female, aged from 48 to 83 years with a mean age of 61.15 years. The ultrasonographic features of breast mucinous adenocarcinoma were analyzed by high-frequency color Doppler sonography. According to the histopathological classification, they were divided into simple mucinous adenocarcinoma of the breast and mixed mucinous adenocarcinoma of the breast, and VTQ was used to detect the elasticity Characteristic [shear wave velocity (Vs)]. Results The morphology of mucinous adenocarcinoma of breast was irregular, the edge was clearer, the echogenicity of the back was enhanced, the echo inside the tumor was low, there were less microcalcifications in the tumor and less distribution of blood flow in the tumor. The difference between simple and mixed mucinous adenocarcinoma Internal echo uniformity was statistically significant. Vs = (1.15 ± 0.75) m / s in the simple-type mucinous adenocarcinoma and Vs = 4.32 ± 0.63 / s in the mixed-type mucinous adenocarcinoma (P <0.05) (P <0.05). Compared with peripheral and normal mammary tissues, the internal mammary tumors in simple breast mucinous adenocarcinoma group were lower than those in peripheral and normal breast tissues. Conclusion Conventional sonography of mucinous adenocarcinoma of the breast has certain characteristics. The application of VTQ is of further significance in the diagnosis of mucinous adenocarcinoma of the breast.
