目的:评估锥形束CT(CBCT)对面部对称性标记点识别的可靠性,为日后建立面部对称性的正畸三维头影测量分析提供参考。方法:对57例正畸患者,使用mimics10.0软件对其CBCT图像采用多平面重建法(矢状向、水平向和冠状向联合定点)标定28个与面部对称性相关的标记点。本研究共2位观察者,每位重复定点3次,分别间隔2周和4周。计算每个标记点的各个维度坐标值的组间和组内相关系数,以评估观察者内部以及观察者之间的定点的可靠性。结果:蝶鞍影像的中心(S)、枕骨大孔前缘之中点(Ba)、硬腭后部骨棘之尖(PNS)、前颅底筛板正中型同鸡冠的骨性结构的中心点(Cg)、鼻额缝的最前点(N)、左右侧蝶骨小翼的最外侧缘[Antcr(L,R)]、左右侧额骨、鼻骨和泪骨交界处[D’(L,R)]、左右侧眶下孔点[K(L,R)]、左右侧上颌牙槽嵴处最凹点[Maz(L,R)]、左右侧眶下缘之最低点[Or(L,R)]、[Zyg(L,R)]、左右侧髁突的顶点[Cd(L,R)]的可靠性良好,可用于CBCT三维头影测量面部对称性的分析。结论:本研究28个面部对称性相关标记点中有19个标记点的可靠性良好,具有临床实用价值。
Objective: To evaluate the reliability of conical beam CT (CBCT) recognition of facial symmetry points and provide a reference for future orthodontic 3D cephalometric analysis of facial symmetry. Methods: Totally 57 orthodontic patients were examined by mimics 10.0 software to calibrate 28 CBCT images with facial symmetry using multiplanar reconstruction (sagittal, horizontal and coronal joint). A total of 2 observers in this study, each repeated three times, respectively, interval of 2 weeks and 4 weeks. Calculate the inter-group and intra-group correlation coefficients for each dimensional coordinate value of each point to evaluate the reliability of the fixed point between the observer and the observer. Results: The center of the sellae image (S), the midpoint of the anterior margin of the foramen magnum (PNS), the posterior spine of the hard palate (PNS), the center of anterior skull base plate (Cg), forefront of the nasal septum (N), outermost margins of left and right sphenoid wing [Antcr (L, R)], left and right frontal bone, nasal bone and lacrimal junction [D ’ (L, R)], left and right infraorbital foramen points [K (L, R)], left and right maxillary alveolar ridges at the lowest point [Maz , R]], [Zyg (L, R)], the apex [Cd (L, R)] of the left and right condyles has good reliability and can be used to analyze the facial symmetry of CBCT three- Conclusion: The reliability of 19 of the 28 facial symmetry-related markers in this study is good and has clinical value.
