目的 探讨损伤控制理念在创伤性胰腺炎(traumatic pancreatitis, TP)治疗中的应用价值.方法 回顾性分析安徽医科大学第二附属医院2012年6月-2018年6月应用损伤控制理念救治15例TP患者的临床资料.胰腺损伤分级采用美国创伤外科协会(American Association for the Surgery of Trauma, AAST)分级标准.结果 15例TP患者,胰腺损伤AAST分级I级1例,II级9例,III级2例,IV级3例.15例患者中死亡2例,均死于多脏器功能衰竭;1例因社会原因放弃治疗,好转1例,治愈11例.结论 TP的治疗要兼顾创伤和胰腺炎两个方面,损伤控制理念的应用及微创手术可以有效改善预后,降低病死率.“,”Objective To explore the value of damage control concept in the treatment of traumatic pancreatitis (TP). Methods Clinical data of 15 patients with TP treated in damage control concept from June 2012 to June 2018 were an?alyzed retrospectively. The classification of pancreatic injury is based on the American Association for the Surgery of Trauma (AAST) classification standard. Results 15 patients with TP, 1 case with AAST grade I, 9 cases with II grade, 2 cases with III grade and 3 cases with IV grade. Of the 15 patients, 2 cases died of multiple organ failure, one case was abandoned for social reasons, one case was improved, and 11 cases were cured. Conclusion The treatment of TP should consider both trauma and pancreatitis. The application of damage control concept and minimally invasive sur?gery can effectively improve the prognosis and reduce the fatality rate.
