目的观察和评估彩色多普勒超声联合全数字化X线检查对乳腺癌老年患者诊断准确性的影响。方法回顾性分析该院2013年4月至2015年4月术后经病理检查确诊为乳腺癌的患者60例、乳腺良性肿瘤患者40例的相关资料。患者术前均使用全数字化X线检查和彩色多普勒超声检查。并将患者术后的病理诊断情况与两种检查方法的检查结果进行对比分析,统计两种检查方法单用和联合使用的情况下诊断乳腺癌的准确性。结果 100例患者全数字化X线检查的诊断准确性为83.00%,灵敏度为83.33%,特异度为82.50%,阳性预测值为87.72%,阴性预测值为76.74%;经彩色多普勒超声检查,诊断准确性为79.00%,灵敏度为80.00%,特异度为77.50%,阳性预测值为84.21%,阴性预测值为72.09%;两者联合检查的诊断准确性、灵敏度、特异度、阳性预测值、阴性预测值分别提升至91.00%,91.67%,90.00%,93.22%,87.80%。结论彩色多普勒超声与全数字化X线检查联合诊断乳腺癌,能有效弥补全数字化X线检查对肿块囊实性鉴别和血供情况判断不敏感的缺点,提高诊断的整体准确性,降低误诊率和漏诊率。
Objective To observe and evaluate the effect of color Doppler ultrasound combined with digital radiography on the diagnostic accuracy of elderly patients with breast cancer. Methods A retrospective analysis of the hospital from April 2013 to April 2015 postoperative pathological examination confirmed 60 cases of breast cancer patients, benign breast tumor in 40 patients with relevant information. All patients were preoperative digital radiography and color Doppler ultrasound. And the pathological diagnosis of patients after surgery and the two test methods to compare the results of the inspection, statistics, two methods of single and combined use of the diagnosis of breast cancer accuracy. Results The diagnostic accuracy of 100 digital radiography was 83.00%, the sensitivity was 83.33%, the specificity was 82.50%, the positive predictive value was 87.72% and the negative predictive value was 76.74%. The color Doppler ultrasonography, The diagnostic accuracy was 79.00%, the sensitivity was 80.00%, the specificity was 77.50%, the positive predictive value was 84.21% and the negative predictive value was 72.09%. The diagnostic accuracy, sensitivity, specificity, positive predictive value, Negative predictive value increased to 91.00%, 91.67%, 90.00%, 93.22%, 87.80% respectively. Conclusion The combination of color Doppler ultrasonography and digital radiography in the diagnosis of breast cancer can effectively compensate for the shortcomings of full digital X-ray examination in detecting cystic solidity and blood supply, improve the overall diagnostic accuracy and reduce the misdiagnosis Rate and missed diagnosis rate.
