目的:探讨血清胱抑素C(CysC)与β2-微球蛋白(β2-MG)对糖尿病早期肾损伤的临床意义。方法:用乳胶颗粒免疫比浊法测定CysC、β2-MG,用酶法测定肌酐,用SPECT(单光子发射计机算断层显像术)测定肾小球滤过率(GFR)。结果:肾功正常组与健康对照组各指标无差异。肾功不全代偿组只有CysC明显高于健康对照组(P<0.01),而其他指标与对照组无差异。在肾功不全代失偿组CysC升高的程度(P<0.001)也高于β2-MG升高的程度(P<0.01)。结论:作为糖尿病早期肾损伤的标志物,CysC比β2-MG更灵敏更特异,是监测糖尿病肾功能的理想指标。
Objective: To investigate the clinical significance of serum cystatin C (CysC) and β2-microglobulin (β2-MG) on early diabetic renal injury. Methods: CysC and β2-MG were measured by latex particle turbidimetric method. Creatinine was measured by enzymatic method. Glomerular filtration rate (GFR) was measured by SPECT (single photon emission computed tomography). Results: There was no difference in each index of normal renal function group and healthy control group. Only CysC in compensated renal insufficiency group was significantly higher than that in healthy control group (P <0.01), while other indexes were not different from those in control group. The level of CysC increased (P <0.001) in patients with decompensated renal insufficiency was also higher than that of β2-MG (P <0.01). Conclusions: As a marker of early renal injury in diabetes, CysC is more sensitive and specific than β2-MG, which is an ideal indicator to monitor diabetic renal function.
