目的观察T2DM患者艾塞那肽治疗前后血清成纤维细胞生长因子-21(FGF-21)水平的变化。方法本研究是一项在多中心临床试验受试者中进行的探索性研究,入选二甲双胍、胰岛素促泌剂单药或联合治疗血糖控制不佳的T2DM患者,予艾塞那肽治疗16周。分别于治疗前后测定血清FGF-21水平及相关代谢指标。结果共入组受试者240例,其中,203例完成干预和随访,平均年龄(49.5±9.9)岁,糖尿病病程4.75(2.33,8.25)年。治疗前平均HbA_1c和体重分别为(8.21±0.90)%和(79.93±15.00)kg,治疗后分别为(7.02±1.05)%和(77.94±14.76)kg(P<0.001)。治疗前后血清FGF-21水平分别为143.97(99.90,213.90)和151.27(108.06,224.14)pg/ml(P<0.05)。结论在二甲双胍、胰岛素促泌剂单药或联合治疗血糖控制不佳的T2DM患者中,添加艾塞那肽治疗16周不仅能有降糖和减重效应,还可升高血清FGF-21。
Objective To observe the changes of serum fibroblast growth factor-21 (FGF-21) levels in patients with T2DM before and after exenatide treatment. METHODS: This study, an exploratory study in multicenter clinical trials, enrolled metformin, insulin secretagogues single agent, or combination therapy in patients with poorly controlled glycemic control of T2DM for exenatide for 16 weeks. Serum FGF-21 levels and related metabolic parameters were measured before and after treatment. Results A total of 240 subjects were enrolled. Among them, 203 patients completed the intervention and follow-up. The average age was (49.5 ± 9.9) years and the duration of diabetes was 4.75 (2.33 and 8.25) years. The mean HbA 1c and body weight before treatment were (8.21 ± 0.90)% and (79.93 ± 15.00) kg, respectively, and were 7.02 ± 1.05% and 77.94 ± 14.76 kg, respectively (P <0.001) after treatment. Serum FGF-21 levels before and after treatment were 143.97 (99.90, 213.90) and 151.27 (108.06, 224.14) pg / ml respectively (P <0.05). CONCLUSIONS: Exenatide treatment with exenatide for 16 weeks not only had hypoglycemic and weight loss effects but also increased serum FGF-21 in patients with T2DM treated with metformin or insulin secretagogues alone or in combination therapy.
