例1 男,17岁。因水肿3个月,少尿3天,于1994年11月23日入院。缘1994年8月患“上感”,5天后出现眼睑水肿,尿蛋白(卌),但BUN、Scr正常,用强的松后水肿消退,尿蛋白转阴。但骤停强的松3天后再现水肿,尿量锐减10ml/d。入院时BP13/10kPa(97.5/75mmHg),全身水肿(卅),余(-)。尿:蛋白(卌),比重1.012。血:白蛋白18.4g/L,BUN17.45～26.43mmol/L,Scr315.5～971.0μmol/L,胆固醇9.10mmol/L,肾彩超:右肾12.5cm×6.6cm×6.1cm,左肾12.7cm×6.8cm×5.7cm,两侧肾静脉未见血栓形成。入院后静注速尿、地塞米松,静输白蛋白、血浆,隔日血透4次,10天后日尿量400～3500ml,BUN7.25mmol/L,Scr54.5μmol/L,续用强的松11天后,尿蛋白(-),定量0.08g/d。
