目的应用超声检测评估2型糖尿病患者颈动脉流体动力学和生理学指标与脑梗死发生的关系。方法选择2型糖尿病患者80例,其中合并急性脑梗死的患者40例(A组),不合并急性脑梗死的患者40例(B组),应用超声检查颈动脉内-中膜厚度(IMT)及斑块,检测流体动力学和生理学相关指标。结果 A组IMT增厚,斑块检出率显著增加,扩张系数(Dc)、顺应系数(Cc)降低,僵硬度(β)增高,收缩期峰值血流速度(PSV)、舒张末期血流速度(EDV)、平均时间平均血流速度(TAVM)、平均血流速度(MV)降低,与B组比较,差异有统计学意义(P<0.05)。结论超声检测颈动脉IMT,评估颈动脉管壁生理学指标、血流动力学指标的改变,可作为反映糖尿病患者颈动脉粥样硬化的指标,对指导糖尿病患者脑梗死的预防和治疗有一定的临床意义。
Objective To evaluate the relationship between carotid hemodynamics and physiology in patients with type 2 diabetes mellitus and the occurrence of cerebral infarction by using ultrasonic testing. Methods Eighty patients with type 2 diabetes mellitus were selected. Forty patients with acute cerebral infarction (group A) and 40 patients without acute cerebral infarction (group B), the carotid intima - media thickness (IMT) And plaques, testing fluid dynamics and physiology related indicators. Results In group A, the IMT was thickened, the plaque detection rate was significantly increased, the dilatation coefficient (Dc), compliance coefficient (Cc), stiffness (β) increased, systolic peak flow velocity (PSV), end diastolic flow velocity (EDV), average time mean blood flow velocity (TAVM) and average blood flow velocity (MV) decreased. Compared with group B, the difference was statistically significant (P <0.05). Conclusion Ultrasound detection of carotid artery IMT, assessment of carotid artery wall physiological indicators, hemodynamic indicators of change, can be used as a marker of carotid atherosclerosis in diabetic patients with indicators, to guide the prevention and treatment of cerebral infarction in patients with diabetes have a certain clinical significance.
