目的 探讨临床感染性标本中产超广谱 β-内酰胺酶(ESBL )大肠埃希菌的阳性率及耐药性 ,以指导临床合理用药 .方法 ESBL 的检测和药敏试验均采用 K- B法、E- test法和 Vitek微生物自动分析仪进行 .结果 大肠埃希菌 12 4株产超广谱β-内酰胺酶者 2 1株 ,阳性率为 16 .9% . 19种抗生素中 ,其对头孢哌酮 /舒巴坦、亚胺培南耐药率分别为 4.8% ,9.6 % ;头孢他啶、头孢西汀和丁胺卡那均为 2 3.8% ;对其他抗生素的耐药率为 6 6 .0 %~ 10 0 .0 % .结论 产超广谱 β-内酰胺酶的大肠埃希菌株对大多数常用抗生素耐药率均较高 ,值得重视 ,其治疗首选药物为头孢哌酮 /舒巴坦和亚胺培南
Objective To investigate the positive rate and drug resistance of extended-spectrum β-lactamases (ESBL) -producing Escherichia coli in clinically infective specimens to guide clinical rational use.Methods ESBL detection and drug susceptibility tests were performed using the K-B method , E-test method and Vitek automatic analyzer.Results 21 strains of Escherichia coli producing 12 strains of extended-spectrum β-lactamase, the positive rate was 16.9%. Among the 19 kinds of antibiotics, The antibacterial rates of cefoperazone / sulbactam and imipenem were 4.8% and 9.6%, respectively. The rates of ceftazidime, cefoxitin and amikacin were both 3.8%, and those of other antibiotics were 66%. 0% ~ 100.0% .Conclusion Escherichia coli strains producing extended-spectrum β-lactamases are the most commonly used antibiotics resistant rate are worthy of attention, the treatment of choice for cefoperazone / Tan and imipenem
