目的探讨多排螺旋CT(HD750)血管造影在下肢动脉人工血管旁路移植术后中长期随访的价值。方法 2004年7月至2011年2月对连续108例下肢动脉人工血管旁路移植术后3年以上患者进行了多排螺旋CT(HD750)血管造影检查,采用容积显示(VR);最大密度投影(MIP);曲面重建(CPR);仿真内窥镜(VE)及高级血管分析(AVA)等多种后处理技术并结合原始横轴位图像进行综合分析。结果多种后处理图像相结合共显示移植血管147条,可以准确评估人工移植血管的位置及形态、吻合口处病变情况、移植血管腔内狭窄程度、侧支循环开放和并发症等。其中53例患者(64条移植血管)与同期DSA检查进行了比较研究。全部病例MDCTA检查结果均符合临床诊断要求。结论多排螺旋CT(HD750)血管造影术对人工移植血管的评价是安全、可靠和准确的,可作为目前在下肢动脉病变人工血管旁路移植术后中长期随访的首选影像诊断方法。
Objective To investigate the long-term follow-up of multislice helical CT (HD750) angiography in the lower extremity artery bypass graft. Methods From July 2004 to February 2011, multislice helical CT (HD750) angiography was performed in 108 consecutive patients with lower extremity arterial bypass grafting more than 3 years. Volumetric display (VR), maximum density projection (MIP); surface reconstruction (CPR); virtual endoscopy (VE) and advanced vascular analysis (AVA) and other post-processing techniques combined with the original horizontal axial image for comprehensive analysis. Results A total of 147 post-transplantation images were combined to show the location and morphology of the grafts, the anastomotic lesion, the degree of intraluminal stenosis, the collateral circulation and complications. Fifty-three of these patients (64 transplanted vessels) were compared with concurrent DSA. All cases MDCTA test results are in line with clinical diagnosis requirements. Conclusion Multi-slice spiral CT (HD750) angiography is safe, reliable and accurate for the evaluation of artificial allograft vessels. It can be used as the first choice of imaging diagnosis in middle and long-term follow-up of artificial blood vessel bypass grafting.
