[目的]观察温化痰汤联合西药治疗肺癌胸腔积液疗效。[方法]使用随机平行对照方法,将36例门诊患者按随机数字表法分为两组。对照组16例抽液,降压迫症状,改善呼吸,大量胸水者每周抽液2~3次,直至胸水完消失,首次抽液量不要超过700mL,以后每次抽液量不应超过1000mL。中毒严重、大量胸水,强的松30mg/次,3次/d,病情好转后逐渐减量。CE方案:CBP300mg/m2静滴,1次/d,VP-16,100mg/次,2次/d,静滴。治疗组20例温化痰汤(半夏5g,天南星8g,禹白附7g,白芥子11g,皂荚14g,旋复花16g,白前20g,猫爪草、白术各13g,陈皮18g)。1剂/d,2000mL水煎至250mL,1次/d。西药治疗同对照组。连续治疗7d为1疗程。观测临床症状、不良反应。连续治疗2疗程,判定疗效。[结果]治疗组痊愈8例,显效5例,有效6例,无效1例,总有效率95.00%。对照组痊愈6例,显效2例,有效4例,无效4例,总有效率75.00%。治疗组疗效优于对照组(P<0.05)。[结论]温化痰汤联合西药治疗肺癌胸腔积液效果显著,值得推广。
[Objective] To observe the curative effect of warming Huatang Decoction combined with Western medicine on pleural effusion of lung cancer. [Methods] Using randomized parallel control method, 36 outpatients were divided into two groups according to random number table method. The treatment group, 20 cases of warming phlegm soup (Pinellia 5g, Aconitum 8g, Yu Bai attached 7g, Mustard Seed 11g, Acacia 14g, spin flower 16g, white before 20g, cat’s claw, Atractylodes each 13g, orange peel 18g). 1 dose / d, 2000mL decoction to 250mL, 1 time / d. Western medicine treatment with the control group. Continuous treatment 7d for a course of treatment. Observed clinical symptoms, adverse reactions. Continuous treatment of 2 courses to determine the efficacy. [Result] In the treatment group, 8 cases were cured, 5 cases were markedly effective, 6 cases were effective, 1 case was ineffective and the total effective rate was 95.00%. The control group cured 6 cases, 2 cases markedly effective in 4 cases, 4 cases, the total effective rate was 75.00%. The treatment group was better than the control group (P <0.05). [Conclusion] Wen Huatan Decoction combined with western medicine has significant effect in treating pleural effusion of lung cancer and is worth popularizing.
