目的探讨多层螺旋CT血管成像(MSCTA)诊断上肢无脉症的方法和临床应用价值。资料与方法对34例临床诊断无脉症的患者行MSCTA检查,所获数据传输至AW4.2工作站,进行最大密度投影(MIP)、曲面重组(CPR)、容积再现(VR)及高级血管分析(AVA)等多种后处理并结合原始图像进行分析。结果34例的血管重组图像对无脉症均有特征显示,动脉硬化闭塞症24例,多发性大动脉炎4例,外伤后动脉栓塞2例,动脉内癌栓2例,锁骨下静脉栓塞不伴动脉疾患2例。结论MSCTA对上肢无脉症的显示有独特的优势,将逐渐成为诊断血管病变及评估手术疗效的首选影像学检查方法。
Objective To investigate the method and clinical value of multi-slice spiral CT angiography (MSCTA) in the diagnosis of aortic pulselessness in upper extremity. Materials and Methods 34 patients with clinically diagnosed pulse-free disease underwent MSCTA and the data were transmitted to AW4.2 workstation for maximum density projection (MIP), surface reconstruction (CPR), volumetric reconstruction (VR) and advanced vascular analysis (AVA) and other post-processing combined with the original image analysis. Results The images of 34 vessels showed no signs of pulseless disease. 24 cases of arteriosclerosis obliterans, 4 cases of multiple arteritis, 2 cases of posttraumatic arterial embolism, 2 cases of intra-arterial tumor embolism, and no subclavian vein thrombosis 2 cases of arterial disease. Conclusion MSCTA has unique advantages for the display of a pulseless upper extremity and will gradually become the first choice of radiological examination to diagnose vascular lesions and evaluate the curative effect of the surgery.
