目的:探讨不同手术方式对绝经前乳腺癌患者术后不同时期生活质量的影响。方法:采用FACT-B(V 4.0)中文版对2008年7月—2015年7月在东南大学附属中大医院进行乳腺癌手术且术后需进行内分泌治疗的绝经前患者进行问卷调查,其中改良根治组患者157例,保留乳房的乳腺癌切除术组患者86例,分别进行术后1年、2~5年、6~7年生活质量统计学对比分析。结果:保留乳房的乳腺癌切除术组患者术后生存质量优于改良根治组患者,且差异有统计学意义(P<0.05);术后1年内两组患者生存质量差异无统计学意义(P>0.05);术后2~5年、6~7年保乳组患者生活质量均优于改良根治组(均P<0.05)。结论:接受保留乳房的乳腺癌切除术患者能获得比接受传统乳腺癌改良根治术患者更佳的生活质量,且这种优势在术后1年后区别更为明显。
Objective: To investigate the effect of different surgical methods on the quality of life of postmenopausal women with breast cancer at different stages after operation. Methods: The FACT-B (V 4.0) Chinese version was used to conduct a questionnaire survey of premenopausal patients who underwent breast cancer surgery at Zhongda Hospital Affiliated to Southeast University from July 2008 to July 2015 and who needed endocrine therapy after surgery. Among them, 157 patients in the radical group and 86 patients in the breast conserving group undergoing breast resection were analyzed statistically in 1 year, 2 to 5 years and 6 to 7 years respectively. Results: The postoperative quality of life of patients with breast cancer who underwent breast conserving surgery was better than that of radical cure patients (P <0.05). There was no significant difference in quality of life between the two groups after 1 year (P > 0.05). The quality of life of breast-conserving group was better than that of modified radical-treatment group (all P <0.05) after 2-5 years and 6-7 years. CONCLUSIONS: Patients who underwent breast-conserving resection achieved a better quality of life than those who underwent modified radical mastectomy. This advantage was more evident one year after surgery.
