目的:评价肝脏螺旋CT动脉造影(SCTAL)对肝肿瘤的诊断价值。方法:对20例肝细胞性肝癌(HCC)和10例肝内转移癌患者作SCTAL:经皮穿刺肱动脉,将5-FCobra导管送入肝动脉,以0.3~1.5ml/s的速度经导管注入浓度为300mgI/ml的造影剂15~30ml,在开始注入造影剂6~10s后用螺旋CT行全肝扫描。结果:所有肿瘤病灶均显著增强,肿瘤的实体部分CT值为105~413HU,明显高于邻近肝组织。HCC灶多呈不均匀增强,转移灶多呈环形增强,而直径为3~10mm的病灶呈均匀增强。结论:与常规肝脏CT增强扫描相比,SCTAL可发现更多的多血供病灶,能真实显示肝肿瘤的肝动脉血供情况。
Objective: To evaluate the diagnostic value of hepatic spiral CT angiography (SCTAL) in liver tumors. METHODS: SCTAL was performed on 20 patients with hepatocellular carcinoma (HCC) and 10 patients with intrahepatic metastases. Percutaneous puncture of the iliac arteries was performed. 5-F Cobra catheters were delivered to the hepatic artery at a dose of 0.3 to 1.5 ml/s. At a speed of 15 to 30 ml of contrast medium at a concentration of 300 mgI/ml was injected through the catheter, and hepatic scan was performed with helical CT 6 to 10 seconds after the start of contrast agent injection. RESULTS: All tumor lesions were significantly enhanced. The CT value of the solid part of the tumor was 105 to 413 HU, which was significantly higher than that of the adjacent liver tissue. Most of the HCC lesions were unevenly enhanced, and the metastatic lesions were mostly annularly enhanced. The lesions with a diameter of 3 to 10 mm were uniformly enhanced. Conclusion: Compared with conventional liver enhanced CT scan, SCTAL can find more blood supply lesions and can truly show the liver artery blood supply of liver tumors.
