目的探讨社区-医院一体化管理模式对糖尿病患者的影响。方法将本社区卫生服务中心就诊的140例糖尿病患者随机分为对照组和观察组各70例,前者采取普通管理,后者给予社区-医院一体化管理模式,对比2组患者糖化血红蛋白及空腹血糖水平。结果对照组患者给予常规管理,半年后平均空腹血糖为(7.38±1.87)mmol/L,观察组给予社区-医院一体化管理模式半年后患者平均空腹血糖水平为(5.84±0.53)mmol/L,2组比较差异有统计学意义(P=0.023);对照组患者半年后糖化血红蛋白水平为(8.12±2.89)mmol/L,观察组半年后患者糖化血红蛋白水平为(6.34±1.35)mmol/L,2组比较差异有统计学意义(P=0.031)。结论社区-医院一体化管理模式能明显改善糖尿病患者不良饮食习惯,并且能有效降低血糖及糖化血红蛋白水平。
Objective To explore the impact of community-hospital integrated management on patients with diabetes mellitus. Methods A total of 140 diabetic patients treated by this community health service center were randomly divided into control group and observation group with 70 cases each. The former adopted general management and the latter community-hospital integrated management mode. The patients in two groups were compared on HbA1c and fasting blood glucose Level. Results The control group was given routine management. The average fasting blood glucose level was (7.38 ± 1.87) mmol / L after six months. The mean fasting blood glucose level in the community-hospital integrated management model in the observation group was (5.84 ± 0.53) mmol / L after six months, The level of HbA1c in the control group was (8.12 ± 2.89) mmol / L after six months, and the level of HbA1c in the control group was (6.34 ± 1.35) mmol / L after six months. The difference between the two groups was statistically significant (P = 0.023) The difference between the two groups was statistically significant (P = 0.031). Conclusion The community-hospital integrated management model can significantly improve the poor eating habits of diabetic patients, and can effectively reduce blood glucose and HbA1c levels.
