栓塞性脑梗死(embolic cerebral infarction,ECI)的确诊常很困难,因许多诊断标准系暂定的,除非患者有多发性同时存在的脑梗塞及全身栓塞的证据,或血管造影发现栓子。本文报告3例无轻偏瘫的皮质性失语(global aphasia GA)可为提示 ECI 的体征。例1:男,50岁,右利者,精神错乱2天入院。检查神志机敏,但不能跟随听和写的命令、说物名、读,用极大努力仅能说“不”或“是”。右同向视野缺损,外伸右臂稍倾斜但无轻偏瘫的证据,腱反射、蹠反应、姿势均正常,感觉检查困难。有多血症和手足
The diagnosis of embolic cerebral infarction (ECI) is often difficult because many diagnostic criteria are tentative unless the patient has multiple concurrent evidence of cerebral infarction and systemic embolism or angiography findings of emboli. This article reports 3 cases of aphasia without hemiplegia aphasia (global aphasia GA) may be indicative of ECI. Example 1: Male, 50 years old, right-winger, mentally disorganized 2 days admitted.
