目的 研究肺癌组织中微血管密度 (micro vasculardensity ,MVD)与肺癌病理生理特征及预后的关系。方法 采用免疫组化LSAB法 ,检测 114例肺癌组织、3 0例肺良性病变组织中的MVD。结果 ①肺癌组织中MVD( 3 0 .3 7± 10 .5 6)显著高于肺良性病变组织 ( 10 .2 3± 6.2 9) (P <0 .0 1) ;②MVD与肺癌原发肿瘤大小 ,淋巴结转移状态 ,P TNM分期 ,细胞分化程度等均有密切关系 (P <0 .0 1) ,而与肺癌患者性别、年龄、吸烟史、肺癌原发部位、组织学类型等均无明显关系 (P >0 .0 5 ) ;③肺癌组织中MVD高密度(MVD≥ 3 0 )组患者 5年生存率 ( 17.2 7% )明显低于低密度 (MVD <3 0 )组 5年生存率 ( 5 2 .74% ) (P <0 .0 1) ;④MVD与肺癌患者术后生存期呈显著负相关 (r =-0 .5 2 1,P <0 .0 1) ;⑤多因素COX模型分析发现 :P TNM分期和MVD是判断肺癌预后最有价值的指标。结论 MVD在肺癌的发生、发展、转移中起重要作用 ,可作为肺癌生物学行为的一项评估指标 ,检测肺癌中MVD有助于预测肺癌预后及指导术后多学科综合治疗。
Objective To study the relationship between microvessel density (MVD) and pathophysiological characteristics and prognosis of lung cancer. Methods Immunohistochemical LSAB was used to detect MVD in 114 cases of lung cancer and 30 cases of benign pulmonary lesions. Results 1 The MVD in lung cancer tissues was significantly higher than that in lung benign tissues (0.332±6.29) (0.37±10.56) (P < 0.01); 2MVD was the primary tumor size of lung cancer. The lymph node metastasis status, P TNM stage, and cell differentiation were closely related (P < 0.01), but there was no significant relationship with gender, age, smoking history, primary site of lung cancer, histological type, etc. in patients with lung cancer. (P > 0.05) ;3 The 5-year survival rate (17.27%) of patients with MVD with high MVD (MVD ≥ 30) was significantly lower than that with low-density (MVD <30). 5 2 .74% ) (P < 0.01); 4MVD was significantly negatively correlated with postoperative survival in patients with lung cancer (r = -0.521, P <0.01); 5 multifactorial COX model The analysis found that: P TNM stage and MVD are the most valuable indicators to determine the prognosis of lung cancer. Conclusion MVD plays an important role in the occurrence, development and metastasis of lung cancer. It can be used as an evaluation index of lung cancer biological behavior. Detecting MVD in lung cancer can help predict the prognosis of lung cancer and guide postoperative multidisciplinary treatment.
