目的检测孕妇样本CMV-IgG抗体亲和力,对排除和确认原发性巨细胞病毒(CMV)感染的性能进行分析,为孕妇CMV感染诊断提供参考依据。方法收集符合要求的114例血清样本,根据血清学状态分为原发感染组(第1组)、既往感染组(第2组)以及无血清转换记录的CMV感染待定组(第3组),测定样本CMVIgG抗体亲和力,根据第1组和第2组结果分析该方法排除或确认原发性CMV感染的性能,并通过测定无血清转换记录的样本CMV-IgG亲和力来说明该方法在临床检测中的意义。结果第1组CMV-IgG抗体亲和力结果显示27例(93.1%,27/29)孕妇的AI处于20%~45%,1例孕妇CMV-IgG抗体AI>50%。第2组41例(91.1%)孕妇AI>50%,35例(77.8%)孕妇AI>60%,显示高亲和力。第3组12例(30.0%)孕妇AI<45%,2例(5.0%)孕妇CMV-IgG抗体亲和力中等(45.0%~55.0%),26例(65.0%)孕妇AI>60%。结论 CMV-IgG亲和力测定确认和排除CMV近期感染的性能良好、提高了血清学诊断,能够有效防止侵入性产前诊断。
Objective To detect the affinity of CMV-IgG antibody in pregnant women and to analyze the performance of excluding and confirming the infection of cytomegalovirus (CMV), so as to provide a reference for the diagnosis of CMV infection in pregnant women. Methods A total of 114 serum samples were collected and divided into primary infection group (group 1), previous infection group (group 2) and CMV infection-undetermined group (group 3) according to serological status. The sample was tested for CMVIgG antibody affinity and analyzed according to the results of Panels 1 and 2 to exclude or confirm the performance of the primary CMV infection and to demonstrate that the method is clinically tested by measuring the affinity of the sample CMV-IgG for seroconversion-free samples Meaning. Results The first group of CMV-IgG antibody affinity results showed that the AI of pregnant women in 27 cases (93.1%, 27/29) was between 20% and 45%, and that of CMV-IgG antibody in one pregnant woman was> 50%. Group 2 41 (91.1%) pregnant women AI> 50%, 35 cases (77.8%) pregnant women AI> 60%, showing high affinity. The AI of pregnant women was less than 45% in 12 cases (30.0%) in group 3, and moderate (45.0% ~ 55.0%) in 2 pregnant women (5.0%). The AI of pregnant women was more than 60% in 26 cases (65.0%). Conclusion The CMV-IgG affinity assay confirms and excludes the recent CMV infection and improves serological diagnosis, which can effectively prevent invasive prenatal diagnosis.
