目的 :探讨丙型肝炎病毒 (hepatitis C virus,HCV)基因型及 HCV RNA量与干扰素 (interferon,IFN)疗效的关系。方法 :应用特异引物聚合酶链反应 (polym erase chain reaction,PCR)法并利用信号引物能量转移技术对2 0例慢性丙型肝炎患者进行 HCV基因分型及 IFN治疗前后血清 HCV RNA定量检测。结果 :HCV- 型患者对IFN治疗的完全应答率高于 HCV- 型 (P<0 .0 5 ) ;治疗前血清 HCV RNA低水平者 ,完全应答率高 ,且完全应答组血清 HCV RNA量明显低于部分应答及无应答组 (P<0 .0 1) ;治疗结束时 3组血清 HCV RNA量较治疗前均有明显下降 ,但停药后半年 ,部分应答及无应答组血清 HCV RNA量均回升至治疗前水平 ,仅完全应答组保持治疗结束时的低水平状态。结论 :治疗前血清 HCV RNA量及 HCV基因型可作为预测 IFN疗效的参考指标 ;治疗及观察过程中血清 HCV RNA量的变化对 IFN疗效评价有重要意义。
Objective: To investigate the relationship between hepatitis C virus (HCV) genotypes and the amount of HCV RNA and the efficacy of interferon (IFN). Methods: The genotypes of HCV in 20 chronic hepatitis C patients and the quantitative detection of serum HCV RNA before and after treatment with IFN were detected by polymerase chain reaction (PCR) and signal primer energy transfer (PCR). Results: The complete response rate of HCV-treated patients was higher than that of HCV-treated patients (P <0.05). Before treatment, patients with low serum HCV RNA had high complete response rate and HCV RNA level in the complete response group (P <0.01). At the end of treatment, the serum HCV RNA levels of three groups were significantly decreased compared with those before treatment, but the levels of HCV RNA in partial response and non-response groups All rose back to their pre-treatment level, with only the complete response group remaining in the low state at the end of treatment. CONCLUSION: The serum HCV RNA and HCV genotype before treatment can be used as a reference index to predict the efficacy of IFN. The changes of serum HCV RNA in the course of treatment and observation are of great significance for evaluating the efficacy of IFN.
